摘要
目的探讨舒张压与80岁以上高血压病人心力衰竭的关系。方法选取我院干部病房一科和心内科170例年龄≥80岁的老年高血压病人,均询问病史,行常规生化检查和动态血压监测;将170例病人分成高血压非心力衰竭组、高血压心力衰竭组2组。分析2组间临床指标以及动态血压监测的差异,并采用Logistic回归分析心力衰竭的危险因素。结果 2组一般资料比较差异均无统计学意义(P>0.05),2组动态血压比较,24 h舒张压(24h DBP)、全天脉压(24h PP)、24 h舒张压负荷(24h DBPL)、白天舒张压(DDBP)、白天脉压(DPP)差异有统计学意义(P<0.05);进一步行Logistic回归分析结果发现,DDBP为心力衰竭(P<0.05)的独立预测因子。结论 DDBP与心力衰竭的发生具有相关性。
Objective To investigate the correlation of diastolic blood pressure（ DBP） with heart failure（ HF）. Methods One hundred and seventy patients aged 80 years and over with hypertension were selected and divided into the non-heart failure group and the heart failure group. All patients performed routine biochemical examination and dynamic blood pressure monitoring. Correlation of blood pressure with heart failure was analyzed. Results The results of clinical data had no significant difference between the two groups（ P〉0. 05）. 24 h diastolic blood pressure,24 h pulse pressure,24 h diastolic blood pressure load,day diastolic blood pressure（ DDBP）,day pulse pressure showed statistical difference between the two groups（ P〈0. 05）. Logistic regression showed that DDBP was associated with HF（ P〈0. 05）. Conclusions DDBP is the independent risk factor for HF.
