摘要
目的探讨老年甲状腺癌患者进行^(131)I治疗对白细胞、粒细胞、单核细胞及淋巴细胞的影响。方法选取接受^(131)I治疗的老年甲状腺癌患者90例,对比^(131)I治疗前后患者的白细胞、粒细胞、单核细胞及淋巴细胞水平变化。根据病理类型,将纳入患者分为乳头状癌组(64例)和滤泡状癌组(26例);根据^(131)I治疗剂量,将纳入患者分为低剂量组(100m Ci,58例)和高剂量组(150 m Ci,32例)。结果治疗后10天,乳头状癌组和滤泡状癌组患者白细胞、粒细胞、单核细胞、淋巴细胞水平比较,差异无统计学意义。治疗后10天,乳头状癌组和滤泡状癌组患者白细胞、粒细胞水平较治疗前上升;淋巴细胞水平较治疗前下降;单核细胞水平治疗前后未明显改变。治疗后10天,低剂量组和高剂量组患者白细胞、粒细胞、单核细胞、淋巴细胞水平比较,差异无统计学意义。治疗后10天,低剂量组和高剂量组患者白细胞、粒细胞水平较治疗前上升;淋巴细胞水平较治疗前下降;单核细胞水平治疗前后未明显改变。治疗后3个月所有患者的白细胞、粒细胞、单核细胞和淋巴细胞水平均恢复正常。结论^(131)I治疗老年甲状腺癌,短期内会导致患者外周血白细胞、粒细胞水平上升,淋巴细胞水平下降,单核细胞水平基本无改变,这一改变为一过性,不受甲状腺癌病理类型、^(131)I治疗剂量的影响。治疗3个月后,患者外周血各项指标恢复正常。
Objective To investigate the effects of 131I on white blood cells ,granulocytes ,monocytes and lymphocytes in elderly patients with thyroid cancer. Methods 90 cases of elderly patients with thyroid cancer treated with 131I were selected. White blood cells, nentrophils, monocytes and lymphocytes were analyzed before and after 131I treatment. According to pathology types, they were divided into papillary carcinoma group(64 cases) and follicular carcinoma group(26 cases) ;according to 131I do- ses,they were divided into low dose group ( 100 mCi,58 cases) and high dose group ( 150 mCi,32 cases). Results After 10 days of 1hi, there were no significant differences in white blood ceils, granulocytes, monocytes and lymphocytes between papillary carcinoma group and follicular carcinoma group. After 10 days of treatment, white blood cells and neutrophils in papillary carcino- ma group and follicular carcinoma group were increased than before;lymphocytes were decreased;no significant changes in mono- cytes. After 10 days of treatment,the differences of white blood cells, granulocytes, monocytes and lymphocytes between low dose group and high dose group were not statistically significant. After 10 days of treatment ,the white blood cells and nentrophils in low dose group and high dose group increased than before;lymphocytes were decreased;monocytes did not change significantly. 3 months after 13~I, the levels of leukocytes, granulocytes, monocytes and lymphocytes in all patients had been recovered. Conclusion tnI can result in hematological changes in elderly patients with thyroid cancer in early stage, white blood cells and granulocytes levels increase, lymphocytes level decrease and no change in monocytes. This change is transient and unaffected by the pathological type and 131I dose. After 3 months, all indexes return to normal.
