摘要
目的:探讨鼻咽癌患者放疗后腮腺血流变化、唾液流速与口腔干燥程度之间的相关性。方法:采用多普勒超声对30例鼻咽癌患者放疗后腮腺实质内小动脉血管收缩期峰值流速(PSV)、血管床阻力状态(RI)进行测定,以纱棉浸唾测定鼻咽癌患者放疗后唾液流速,同时根据放射治疗肿瘤学组(RTOG)"涎腺急性放疗损伤评估表"记录患者多普勒超声检查时口干程度。结果:鼻咽癌患者放疗后,其血流状况与患者口干程度随时间周期存在不同的相关性。放疗结束1~2周内,多普勒超声检测患者腮腺实质内小动脉PSV和RI变化与患者口干程度呈负相关,放疗结束第3周后,多普勒超声检测患者腮腺实质内小动脉PSV和RI变化与患者口干程度呈正相关。检测患者唾液流速与其口干程度一直呈负相关。结论:以多普勒超声检测腮腺血流变化、唾液流速有望成为临床评价鼻咽癌患者放疗后口干程度的一种方法。
Objective:To investigate the correlation between the changes of parotid blood flow,the salivary flow rate and the degree of dry mouth in patients with nasopharyngeal carcinoma after radiotherapy.Methods:The small artery peak systolic velocity(PSV),resistance index(RI)in parotid gland parenchyma in patients with nasopharyngeal carcinoma after radiotherapy were detected by Doppler ultrasonography,the salivary flow rate was detected by cotton yarn of salivary determination,degree of dry mouth was recorded by " salivary gland acute radiation damage assessment".Results:There was different time period correlation between the blood flow and dry mouth in patients with nasopharyngeal carcinoma after radiotherapy.At the end of radiotherapy of nasopharyngeal carcinoma within 1-2 weeks,Doppler ultrasound and RI in patients with PSV artery parotid gland parenchyma changes and degree of dry mouth showed a negative correlation,the end of the third week after radiotherapy PSV and RI showed a positive correlation with degree of dry mouth,salivary flow rate negatively correlated to degree of dry mouth.Conclusion:Doppler ultrasonography in detecting parotid gland blood flow changes,salivary flow rate is expected to become the clinical assessment of the degree of degree of dry mouth in patients with nasopharyngeal carcinoma after radiotherapy.
