高速涡轮牙钻结合锤凿拔除复杂阻生牙临床观察 被引量:4

摘要 目的探讨复杂阻生牙采用高速涡轮牙钻结合锤凿拔除的疗效。方法选取150例于2015年6月~2016年12月期间我院接收的下颌近中阻生智齿患者,随机分为对照组(采用锤凿法拔除阻生牙,n=75)与观察组(采用高速涡轮牙钻结合锤凿拔除阻生牙,n=75),观察并比较两组患者手术时间、术中拔牙窝形变、开口受限度、术后疼痛、肿胀度以及畏惧症、并发症发生情况。结果观察组患者的拔牙窝完整性评分、张口受限度、肿胀度、疼痛度评分显著低于对照组,差异有统计学意义(P<0.05),并且其手术时间相比于对照组显著要短(P<0.05);同时观察组骨板损伤、牙根断裂、干槽症发生率均显著低于对照组(P<0.05);此外,观察组患者畏惧率发生显著低于对照组(P<0.05)。结论对于复杂阻生牙采用高速涡轮钻结合锤凿法拔除,可减轻术后反应,缩短手术时间,降低拔牙畏惧症及并发症的发生,具有推广价值。 Objective To investigate the efficacy of extraction of complex impacted teeth with high speed air turbine handpiece combined with hammer chisel. Methods 150 cases with the mesial impacted teeth in our hospital from June 2015 to December 2016 were selected and randomly divided into control group (The impacted teeth were extracted by hammering,n=75) and observation group (The impacted teeth were extracted by high speed turbine drill combined with hammer and chisel,n=75).The operation time,the extraction fossa deformation,the opening limit, the postoperative pain,the swelling degree, phobias and the complications were observed and compared between the two groups. Results The score of the completeness of the extraction pit,the limit of mouth opening,the degree of swelling and pain score of the observation group were significantly lower than that of the control group (P 〈 0.05).The operation time of the observation group was significantly shorter than that of the control group (P〈0.05).The incidence of bone plate injury,root fracture and dry socket were significantly lower in the observation group than that in the control group (P 〈 0.05);In addition, the phobias rate of the observation group was significantly lower than that of the control group (P 〈 0.05). Conclusion High speed turbine combined with hammer gouge for complicated impacted teeth can reduce the postoperative reaction,shorten the operation time,reduce the occurrence of tooth extraction,fear and complications.
