个体化有氧联合抗阻运动对冠心病并发2型糖尿病患者血糖、血脂以及运动能力的影响 被引量:26

摘要 目的探讨个体化有氧联合抗阻运动对冠状动脉粥样硬化性心脏病(冠心病)并发2型糖尿病(type 2diabetes mellitus,T2DM)患者血糖、血脂等生理指标以及运动能力的影响,并分析其可能机制。方法招募冠心病并发T2DM患者60例纳入研究,并按随机数字分组法随机分为康复组(30例)和对照组(30例)。对照组进行常规药物治疗;康复组在对照组基础上加以根据心肺运动试验(cardiopulmonary exercise test,CPET)测试评估结果,进行强度为40%~60%最大摄氧量(maximal oxygen consumption,VO_2max)的有氧运动,抗阻训练以弹力带测定10 RM为强度。每次联合运动时间30~60 min,每周3次,共12周。12周的治疗前后所有患者测定空腹血糖(FPG)、餐后2 h血糖(PBG)、糖化血红蛋白(Hb A1c)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、脂肪重(FM)、瘦体质量(LBM)和VO_2max。结果对照组治疗前、后各项指标比较,差异无统计学意义(P>0.05)。康复组的血糖、血脂以及运动能力治疗前、后比较,差异有统计学意义(P<0.05)。治疗后康复组各项观察指标明显优于对照组,差异具有统计学意义(P<0.05)。结论个体化有氧联合抗阻运动能够改善冠心病并发T2DM患者血糖、血脂浓度以及运动能力。 Objectives To observe the effects of individualized resistance training and aerobic training on blood glu-cose,lipid and exercise performance in patients with coronary artery disease (CHD) and type 2 diabetes mellitus (T2DM),and explore the possible mechanism. Methods Totally 60 patients of CHD and T2DM were randomly as-signed to rehabilitation group(RG,n=30)and control group(CG,n=30). Patients in CG adopted the routine medica-tion management. On the basis of CG ,patients in RG adopted aerobic training and resistance training whose intensity was set during 40%-60% maximal oxygen consumption (VO2max) according to the cardiopulmonary exercise test (CPET) and at ten-Rep Max (10 RM) measured by the thera-band ,respectively. The combined training was per-formed 30-60 minutes one time ,3 times a week for 12 weeks. All the patients were evaluated with fasting blood-glu-cose(FBG),glycosylated hemoglobin(HBA1c),total cholesterol(TC),triglyceride(TG),low density lipoprotein-cholesterol (LDL-C),fat mass (FM),lean body mass (LBM) and VO2max before and after training. Results There was no significant difference among the physiology index in CG before and after training(P〉0.05). All the index had remarkable differences in RG after training compared with those in RG before training and CG after training (P〈0.05). Conclusions Individualized aerobic training combined with resistance training can significantly improve the concentrations of blood glucose,lipid and exercise performance in patients with CHD and T2MD.
