摘要
目的调查云南省昆明市美沙酮维持治疗病人的治疗成本负荷,为未来药物滥用门诊治疗模式提供数据支持。方法 2015—2016年对昆明市部分美沙酮维持治疗门诊1 000名美沙酮维持治疗病人进行问卷调查,并分析治疗成本,被调查人员按参加美沙酮维持治疗时间分为参加维持治疗半年以内、超过2年和超过5年3组,直接医疗成本包括身体检查费用和治疗费。结果 3个年龄组身体检查费用分别为401元、273元和105元,差异有统计学意义(F=45.855,P<0.05);治疗费分别为321元、256元和154元,差异有统计学意义(F=31.047,P<0.05);间接医疗成本主要为往返交通费,分别为334元、263元和176元,差异有统计学意义(F=34.297,P<0.05);3组被调查人员2016年平均月收入为1 892元、2 203元和2 532元,差异有统计学意义(F=15.780,P<0.05),但均低于2016年昆明市城镇常住居民人均可支配收入(3 114元/月)。结论被调查人员参加美沙酮维持治疗时间越长、其直接和间接医疗成本越低,被调查人员参加美沙酮维持治疗时间越长、收入也越高,但均低于2016年昆明市城镇常住居民人均可支配收入。
Objective To investigate the medical costs of patients for their methadone maintenance treatment（MMT） in Kunming of Yunnan province, and provide data support for future treatment models at substance abuse clinics. Methods A cross-sectional questionnaire survey was conducted in 1 000 patients receiving MMT at 5 MMT clinics in Kunming during2015—2016, and their medical costs were analyzed. The patients were divided into 3 groups as their treatment time（group 1：less than 6 months; group 2： more than 2 years; group 3： more than 5 years） to calculate the direct costs（include physical examination and treatment）. Results The cost of physical examination was 401 Yuan RMB, 273 Yuan RMB and 105 Yuan RMB respectively in group 1, 2 and 3, and there was statistical difference among the three groups（F=45.855, P〈0.05）. The cost of treatment was 321 Yuan RMB, 256 Yuan RMB and 154 Yuan RMB among the three groups with statistical difference（F=34.297, P〈0.05）. The average monthly income of the patients among the 3 groups were 1 892 Yuan RMB, 2 203 Yuan RMB and 2 532 Yuan RBM, respectively, with statistical difference（F=15.780, P〈0.05）. The average monthly income of all three groups was lower than the residents disposable income in Kunming（3 114 Yuan RMB per month） in 2016. Conclusions If patients receive longer MMT, their medical costs of direct and indirect are lower, and the patients get more income,that still lower than the per capita disposable income of residents in Kunming in 2016.
