摘要
目的调查住院患者对现行分级诊疗制度的认知状况并探讨其影响因素。方法采用分层整群抽样的方法,通过河南省人民医院自制"住院患者分级诊疗认知状况及其影响因素调查表",对644例住院患者进行问卷调查,并对回收的问卷进行统计分析。结果本次被调查的患者中,分级诊疗制度的认知率为32.4%;患者的文化程度、职业及医保类型与分级诊疗制度的认知度相关(P<0.05)。对基层医院医务人员的诊疗水平不信任、需重新缴费、转诊手续复杂、医保报销金额差别不大影响患者向下级医院转诊;不同级别医疗机构间医务人员技术水平差别较大、分级诊疗宣传力度不够、转诊流程不通畅影响分级诊疗制度的实施。结论住院患者对分级诊疗制度的认知度较低,且对分级诊疗制度评价较低,应从加大对分级诊疗制度的宣传力度、提高患者参与度、加强基层医院与上级医院的协作、完善分级诊疗配套制度等方面入手,提高患者对分级诊疗制度的认知度,为分级诊疗制度的进一步完善和实施提供依据。
Objective To investigate the cognitive status of inpatients on the current hierarchical diagnosis and treatment system and the influencing factors. Methods By using the method of stratified cluster sampling and the self-made'inpatients grading treatment knowledge and its influential factors questionnaire',644 inpatients were surveyed,and the questionnaires were analyzed after the investigation. Results The survey showed that the diagnosis and classification system recognition rate was32. 4%,and the educational level of the patients,the type of occupation and the type of medical insurance were related to the degree of cognition of the classification system( P<0. 05). Mistrust the primary hospital medical staff level,need to repay,referral complex procedures,and the amount of medical insurance reimbursement of the difference didn't affect the patient to the lower hospital referral. The difference of technical level of different level of medical institutions,lack of propaganda and the bad referral process affected the implementation of hierarchical treatment system. Conclusion The awareness and the evaluation of the classification of treatment system of inpatients are low. Government should increase the level of hierarchical diagnosis and treatment system through increasing the publicity efforts,strengthening the primary hospital and higher hospital collaboration,improving the classification of treatment supporting system and other aspects to improve the patient's awareness of the classification system,so as to provide basis for improving the establishment of hierarchical diagnosis and treatment system and promoting the implementation of grading treatment measures.
