摘要
目的通过分析总结27颗牙根纵裂患牙的临床资料及诊治体会,为临床牙根纵裂的早期诊断和防治提供一定的参考。方法收集已经拔牙证实的27例(27颗)牙根纵裂患者的临床资料,包括患者年龄、性别、牙位、患根分布、相关临床表现、X线根尖片及CBCT表现等;并对相关资料进行分析总结。结果 (1)27例患者年龄39~57岁,平均年龄51.3岁,男女比例为2.8:1;(2)下颌第一磨牙根裂发生率高达50%(13/26),近74.07%(20/27)裂纹发生于磨牙近中根;(3)活髓牙12颗,根管治疗后患牙15颗;临床症状包括咬合疼痛不适、急慢性牙髓炎、根尖周炎和牙周反复肿痛。(4)X线确诊18颗,CBCT确诊5颗,X线与CBCT共同确诊3颗,拔牙后确诊1颗。结论牙根纵裂的发生存在一定易感因素,且临床表现复杂多样。临床医师应加强对该病的认识,以期能做到早发现、早诊断、早治疗;同时应规范临床操作,以预防医源性牙根纵裂的发生。
Objective Analysising and summarizing the clinical data that diagnosis and treatment of 27 patients with vertical root fracture, provides some reference for early diagnosis and prevention of vertical root fracture.Methods Clinical data of 27 patients（27 teeth） with vertical root fractures were collected,which consist patients＇ age,sex,tooth position,with the root distribution,clinical manifestations.X-ray and CBCT manifestations,then analysis these data.Results（1）27 patients were 39～57 years old, with an average age of 51.3 years, and the ratio of male to female was 2.8：1;（2）The incidence of mandibular first molars was 50%（13/26）, and nearly 74.07%（20/27） occurred in the mesial root of molars;（3）vital tooth 12 teeth, 15 teeth after root canal therapy; clinical symptoms include occlusal pain, acute and chronic pulpitis, periapical and periodontal pain repeatedly;（4）X-ray diagnosis of 18, CBCT confirmed the diagnosis of the disease 5 tooth, X-ray and CBCT joint diagnosis of 3, after tooth extraction, confirmed the diagnosis of 1 cases.Conclusion There are some predisposing factors in the occurrence of vertical root fracture, and the clinical manifestations are complex and diverse. Clinicians should strengthen the understanding of the disease in order to achieve early detection, early diagnosis, early treatment; At the same time, we should standardize the clinical operation in order to prevent iatrogenic root fracture.
