摘要
目的探讨手术前通过超声检查对骨关节结核脓肿进行分析定位,以指导手术有效切除骨关节结核脓肿病灶,减少病灶复发。方法选择105例需手术治疗的骨关节结核伴周围脓肿患者,术前均行超声检查,并标记病灶位置,走行及范围,然后术中进行观察,分析超声显像与术中所见的一致性,指导手术切除病灶。结果二维超声可准确显示骨关节结核周围脓肿的位置与范围及周围窦道的走行及部位,彩色多普勒超声可显示骨关节旁及脓肿周围血管的分布及血流性质,与术中所见一致。结论超声可以对骨关节结核周围脓肿及窦道病灶范围位置、走行准确定位,对指导临床制定相应的手术方案具有重要意义。
Objective To explore the surgical examination of bone and joint tuberculosis abscess by ultrasound examination to guide the operation of effective resection of bone and joint tuberculosis abscess lesions to reduce the recurrence of the lesion. Methods 105 cases of bone and joint tuberculosis with surgical treatment of peripheral abscess were selected. Ultrasonography was performed before operation. The location,travel and range of the lesion were recorded. Then,intraoperative observation was performed to analyze the consistency of ultrasonography and intraoperative findings. Surgical resection of the lesion. Results Two-dimensional ultrasound can accurately show the location and extent of the peri-abscess around the bone and joint and the walking and site of the surrounding sinus. Color Doppler ultrasonography can show the distribution of blood vessels around the bone and adjacent abscess and blood flow,consistent with the intraoperative findings. Conclusion Ultrasound can be used to locate the location of abscess and the location of the sinus lesion around the bone and joint tuberculosis,and it is of great significance to guide the clinical development of the corresponding surgical program.
