摘要
目的采用光棒对困难插管经左右口角及正中径路插管效果进行评价。方法选择60例困难插管患者随机分为三组,左口角(L组)、右口角(R组)、正中径路(M组)每组20例,三组麻醉诱导相同,标记插管时光棒进入深度、插管完成时间,比较各组插管便捷性、光棒至甲状软骨长度等。结果 L组弯曲度(106.50±19.06)°,R组弯曲度(106.50±19.06)°,正中径路弯曲度(77.30±7.15)°,光棒弯曲度左右比较差异无统计学意义(P>0.05),左右与正中比较差异有统计学意义(P<0.05)。光棒L组进入深度(12.36±0.67)cm,R组(12.43±0.63)cm,正中径路深度(13.84±0.56)cm,左右比较差异无统计学意义(P>0.05),正中深度与左右径路比较差异有统计学意义(P<0.05)。结论光棒经左右口角插管进入方便快捷,弯曲度小,插管成功率高,而正中入路需要一定的弯曲度。
Objective To evaluate the effectiveness of using light-wand oral tracheal intubation in patients with diffcult airways by different approaches of left,right and median corners of mouth. Methods 60 patients with diffcult airways were randomly divided into 3 groups, with 20 cases in each group. These were group L,group R and group M. After the same method of anesthesia induction, the dapth of ligth-wand was marked, the fnishing time was recorded and the convenience and length from ligth-wand to thyroid were compared between three groups. Results The curvature of light-wand in group L, group R and group M was （106.50±19.06）°, （106.50±19.06）°and （77.30±7.15）°respectively, which has no signifcant difference in group L and group R （P 〉 0.05） and the differences had statisitical signifcance in group L and group R compared with group M（P 〈 0.05）. The depth of light-wand in group L, group R and group M was （12.36±0.67） cm, （12.43±0.63） cm and （13.84±0.56） cm respectively, which has no signifcant difference in group L and group R（P 〉 0.05） and the differences were statisitical signifcance in group L and group R compared with group M（P 〈 0.05）. Conclusion Using light-wand to intubate through left and right corners of mouth are quicker and easier, have small curvature and higher success rate. But it need a certain curvature through median approach.
