摘要
目的:探讨妊娠合并感染性休克患者如何安全、合理使用抗感染药物。方法:介绍1例妊娠合并感染性休克患者抗感染治疗全过程,并从抗感染药物选择、给药剂量调整和用药监护三方面进行分析。结果:患者体温等感染症状逐渐减轻,WBC、NEUT%等感染相关指标逐渐下降,肾功能逐渐恢复,转当地医院继续治疗。结论:(1)妊娠合并感染性休克患者抗感染药物选择既要考虑感染可能病原菌、感染部位、感染严重程度,还要考虑药物对胎儿的安全性,推荐使用FDA妊娠分级为B级的抗感染药物,慎用C级抗感染药物;(2)抗感染药物给药剂量调整除了考虑患者年龄、肝肾功能等因素外,还需要结合妊娠期母体的各项生理变化情况进行调整;(3)抗感染药物用药监护可以从临床疗效、药物不良反应、药物对胎儿安全性等方面进行监护。
Objective： To evaluate safe and rational use of anti-infective drugs in pregnant women with septic shock. Methods： This article describes the entire course of anti-infective therapy of 1 case of pregnancy with septic shock. It was analyzed from the anti-infective drug selection, dosage adjustment and medication monitoring. Results： The temperature and other infection symptoms gradually reduced,WBC,NEUT% and other infection related indicators gradually declined,renal function gradual recovery. Eventually,the patient was transferred to a local hospital for further treatment. Conclusions：（1）The selection of anti-infective drugs in pregnant women with septic shock should consider the possible pathogens of infection,the site of infection,the severity of infection,and the safety of the drugs to the fetus. We recommend the use of FDA pregnancy classification for B class of anti-infection drugs,carefully use of C class.（2）In addition to taking into account the patient＇s age,liver and kidney function,the dosage of anti-infective drugs should be adjusted according to the physiological changes of the mother during pregnancy.（3）The use of anti-infective drugs can be monitored from clinical efficacy,adverse drug reactions,and safety of the drugs to the fetus.
