上海市家庭医生助理员工作能力现状及培训需求研究 被引量:5

摘要 目的了解上海市家庭医生助理员工作能力现状及其培训需求,以期为上海市家庭医生助理员的培训提供参考。方法采用立意抽样法,在上海市最早开展家庭医生助理员试点的闵行区的13家社区卫生服务中心中抽取6家;采用随机抽样法,抽取6家社区卫生服务中心的170例家庭医生助理员为研究对象。于2016年7—8月,采用调查问卷对家庭医生助理员进行调查,问卷主要内容包括:家庭医生助理员的一般情况、对家庭医生工作的促进作用、工作能力评价、工作中遇到的主要困难及需培训的知识和技能。共发放问卷170份,回收有效问卷166份,问卷的有效回收率为97.6%。结果 166例家庭医生助理员中,专业背景为护理专业110例(66.3%),公共卫生专业31例(18.7%),其他专业25例(15.0%)。针对对家庭医生工作的促进作用,不同专业背景的家庭医生助理员在减轻家庭医生工作负担、有助于家庭医生专心诊治、有助于家庭医生节约时间学习/开会/进修、促进医患关系和谐方面比较,差异有统计学意义(P<0.05)。护理、公共卫生、其他专业的家庭医生助理员的工作能力自评得分分别为(7.8±1.9)、(7.1±2.2)、(7.7±1.7),3组比较差异无统计学意义(P>0.05)。针对工作中遇到的主要困难,不同专业背景的家庭医生助理员在工作量太大、医学专业背景差方面比较,差异有统计学意义(P<0.05)。护理专业的家庭医生助理员的主要知识需求为健康心理学知识(72例,65.5%)、常见疾病管理规范(63例,57.3%)、医学基础知识(62例,56.4%);公共卫生专业为健康心理学知识(19例,61.3%)、常见疾病管理规范(17例,54.8%)、营养学基础知识(16例,51.6%);其他专业为医学基础知识(19例,76.0%)、健康心理学知识(18例,72.0%)、营养学基础知识(14例,56.3%)。不同专业背景的家庭医生助理员的知识培训需求比较,差异有统计学意义(P<0.05);其他技能培训需求� Objective To investigate the current work ability and training needs in general practitioner( GP)assistants in Shanghai, with a view to providing a reference for the training of GP assistants in Shanghai. Methods Using purposive sampling,we selected 6 out of the 13 community health service centers( CHSCs) in Minhang District,Shanghai in which services delivered by GP assistants were first carried out on a trial basis in Shanghai. Then,from these 6 CHSCs,we enrolled 170 GP assistants by random sampling. From July to August 2016,we conducted a survey in the GP assistants with a self-developed questionnaire covering the demographic data,positive value of GP assistants' work for the work of the GP,work ability,main challenges encountered at work,and professional knowledge and skills needing to be trained. The survey achieved a response rate of 97. 6%( 166/170). Results Of the 166 final participants,66. 3%( 110/166) majored in nursing,18. 7%( 31/166) majored in public health,and 15. 0%( 25/166) majored in other specialties. The value of GP assistants' work in reducing the workload of the GP,helping the GP concentrate on diagnosis and treatment,increasing the time of the GP for learning,attending meetings and further study,and improving the doctor-patient relationship varied significantly by the major( P〈0. 05). The self-assessment scores of the work ability were( 7. 8 ± 1. 9)( 7. 1 ± 2. 2)( 7. 7 ± 1. 7) for GP assistants majoring in nursing,those majoring in public health and those majoring in other specialties,which were similar in the three groups( P〈0. 05). In view of the main challenges encountered in the work,challenges in self-perceived too heavy workload and self-perceived insufficiency in medical knowledge and skills differed significantly in these three groups( P〈0. 05). The knowledge needing to be trained for GP assistants majoring in nursing mainly included health psychology( 72 cases,65. 5%),common disease management standards( 63 cases,57.
