摘要
碎裂QRS波(fragmented QRS complex,f QRS)的形成机制被认为是心肌瘢痕造成的传导延迟所致,常见于冠心病、心肌梗死患者,也可见于心肌病和先天性心脏病患者。f QRS对致心律失常性右室心肌病的预测及防治、Brugada综合征和非缺血性心肌病的预警均具有重要的临床价值。鉴于目前心源性猝死(sudden cardiac death,SCD)危险分层的主要危险因素及预测参数存在明显的局限性,探索新的无创性心电学检查手段成为亟待解决的问题。f QRS可考虑作为SCD危险分层或积分的重要组成部分之一。
Fragmented QRS complex( f QRS) is regarded to derive from conduction delay caused by myocardial scar,which is commonly seen in patients with coronary heart disease and myocardial infarction,and also in patients suffering from cardiomyopathy and congenital heart disease. Clinically,f QRS plays important roles in the prediction,prevention and treatment of arrhythmogenic right ventricular cardiomyopathy,and the early warning of Brugada syndrome and non-ischemic cardiomyopathy. Since the risk stratification of sudden cardiac death( SCD) is obviously bounded in major risk factors and predictors,it is required to explore new noninvasive electrocardiologic examination method. f QRS can be considered as one of the important parts in the risk stratification or risk score of SCD.
