摘要
目的 评价外伤性视网膜脱离伴巨大裂孔25 G微创玻璃体切除术的效果。方法 回顾性分析外伤性视网膜脱离伴巨大裂孔13例（13只眼）行25 G微创玻璃体切除术的临床资料。术后随访6个月或硅油取出后2～3个月。结果 随访期末视力较术前提高者11例（84.62%），1例视力无变化（7.69%），1例视力较术前下降（7.69%）。术后总体视力较术前提高，差异有统计学意义（t＝3.604，P＝0.004）。随访期间12例（92.31%）视网膜复位，1例（7.69%）硅油取出术后复发视网膜脱离；未发现高眼压者；2例（15.38%）出现并发性白内障；2例（15.38%）发生增生性玻璃体视网膜病变。结论 25 G微创玻璃体切除术治疗外伤性视网膜脱离伴巨大裂孔，术后视网膜复位率高，并发症少。
Objective To evaluate the efficacy of 25 G micro-invasive vitrectomy for the treatment of traumatic retinal detachment associated with giant retinal tear.Methods The data of 13 eyes of 13 patients with tarumatic retinal detachment associated with giant retinal tear underwent 25 G micro-invasive vitrectomy were analyzed retrospectively. The follow-up time was 6 months after operation or 2-3 months after removal of silicone oil. Visual acuity, anatomical success rate and complications were observed.Results At the last follow-up, the postoperative visual acuity was improved in 11 cases （84.62%）, unchanged in 1 case （7.69%） and deteriorated in 1 case （7.69%）. The difference was statistically significant between before and after surgery （t=3.604, P=0.004）. During the follow-up time, retinal reattachment was in 12 cases（92.31%） and recurrent retinal detachment after silicone oil removal in 1 case （7.69%）. Two cases（15.38%） complicated cataract and 2 cases （15.38%） associated with proliferative vitroretinopathy （PVR）. No intraocular hypertension happened.Conclusion The surgery of 25-gauge micro-invasive vitrectomy for the treatment of giant retinal tear associated with retinal detachment can achieve excellent attachment rates and with less complications.
