老年胃癌患者营养风险及营养认知情况调查 被引量:2

摘要 目的调查老年胃癌患者营养风险状况和营养认知水平情况。方法以580例老年胃癌患者为研究对象,通过NRS2002营养风险筛查法对患者进行营养风险评分,通过问卷调查方式对患者营养认知水平进行评分,并统计患者排便时间、肠道排气时间和住院时间。结果在全部580例患者中,高营养风险患者为436例,比例为75.17%,Ⅰ、Ⅱ期患者高营养风险比例(63.60%)显著低于Ⅲ、Ⅳ期患者比例(85.39%),差异有统计学意义(χ2=14.69,P=0.013<0.05);高营养认知水平患者为134例,占23.10%,且患者营养认知评分与患者营养风险评分、患者排便时间、肠道排气时间和住院时间均呈负相关(r=-0.927,-0.462,-0.512和-0.769;P=0.000,0.005,0.001和0.000)。结论目前老年胃癌患者营养风险较高,并且患者对于营养认知水平普遍偏低,而患者营养认知水平的高低对患者营养状况和预后情况的优劣起到重要影响,需进一步加强对老年人群胃癌相关营养知识的培训教育,以提高患者的营养管理意识和能力。 Objective This paper tries to investigate the nutritional risk and nutritional cognitive situation of the elderly gastric with cancer patients. Methods 580 elderly gastric cancer patients were selected as research subjects. Nutritional risk score of all the patients were evaluated by the NRS2002 nutritional risk screening method, and nutritional cognitive situation of all the patients were investigated through the questionnaire. The defecation time, intestinal exhaust time and hospitalization time of patients were statistically analyzed. Results For all the 580 elderly gastric cancer patients, there were 436 patients with high nutritional risk, the ratio was 75.17%, and the proportion of high-risk patients at stage I and II accounted for63.60%, significantly lower than that of patients at stage III and IV of 85.39%, and the difference was statistically significant(χ2=14.69,P=0.0130.05). There were 134 patients with high levels of cognitive nutrition, accounting for 23.10%, and the scores of nutrition cognition were negatively correlated with nutritional risk score, defecation time, intestinal exhaust time and hospitalization time(r=-0.927,-0.462,-0.512 and-0.769;P=0.000, 0.005, 0.001 and 0.000). Conclusion The elderly patients with gastric cancer has high nutritional risk at present, the nutrition knowledge level of the patients is generally low,and the cognitive level of patients plays an important role in the nutritional status and prognosis of patients. The training and education on the elderly gastric cancer of related nutrition knowledge need to be further strengthened so as to improve the patient's nutrition management consciousness and ability.
