摘要
目的观察丹芪祛瘀止痛颗粒治疗气虚血瘀型慢性萎缩性胃炎患者的临床疗效及对血清炎性因子、细胞因子的影响。方法将95例气虚血瘀型慢性萎缩性胃炎患者随机分为对照组48例和治疗组47例,对照组给予胃复春片治疗,治疗组给予丹芪祛瘀止痛颗粒治疗,2组疗程均为12周。观察2组治疗前、治疗后及随访时中医证候积分变化情况,检测2组治疗前后血清炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、肿瘤坏死因子受体I(TNFRI)]及细胞因子[生长激素(GH)、表皮生长因子(EGF)]水平,统计2组临床疗效。结果 2组治疗后及随访时主症积分、次症积分及总积分均明显低于治疗前(P均<0.05),且治疗组各项积分均明显低于对照组(P均<0.05);2组治疗后血清TNF-α、IL-6、TNFRI水平均明显降低(P均<0.05),且治疗组均明显低于对照组(P均<0.05);2组治疗后血清GH、EGF水平均明显升高(P均<0.05),且治疗组均明显高于对照组(P均<0.05)。治疗组总有效率明显高于对照组(P<0.05)。结论丹芪祛瘀止痛颗粒可以明显改善气虚血瘀型慢性萎缩性胃炎患者的临床症状,可能通过下调TNF-α、IL-6、TNFRI水平,提高GH、EGF水平而减轻胃黏膜炎性反应,修复胃黏膜,从而逆转胃黏膜萎缩。
Objective It is to observe the effect of Danqi Quyu Zhitong Granule on inflammatory factors and cell factors in patients with Qi deficiency and blood stasis type chronic atrophic gastritis.Methods 95 cases of patients with with Qi deficiency and blood stasis type chronic atrophic gastritis were divided into control group （48 cases） and treatment group （47 cases）according to the random number table method.The control group was given Weifuchun,however the treatment group were given Danqi Quyu Zhitong Granule,the efficacy of the two groups were evaluated after 12 weeks of treatment.The changes of TCM syndrome score in the two groups before treatment,after treatment and followed up were calculated.The gastroscopy and pathological grading of mucosal atrophy changes and serum inflammatory factors （TNF-α,IL-6,TNFRI） and cell factors （GH,EGF） levels were detected before and after treatment.Results The main symptom scores,the secondary symptom scores and the total symptom scores in the two groups after treatment and during follow-up were significantly decreased than before （all P〈0.05）,and these scores of the treatment group were statistically lower than those of the control group （all P〈0.05）.The levels of TNF-α,IL-6 and TNFRI were decreased in the two groups after treatment （all P〈0.05）,and these indicators of the treatment group were significantly lower than those of the control group （all P〈0.05）.The levels of GH,EGF were increased in the two groups after treatment （all P〈0.05）,and these indicators of the treatment group were significantly higher than those of the control group （all P〈0.05）.The total effective rate of treatment group was significantly higher than that of the control group （P〈0.05）.Conclusion Danqi Quyu Zhitong Granule can improve the clinical symptoms,reduce the TCM syndrome score,the down-regulation of TNFRI,IL-6 and TNF-α level,up-regulation the level of GH and EGF,reducing the gastric mucosal inflammation and repair mucosa,to reverse the
