摘要
目的探讨骨髓涂片对慢性骨髓增殖性疾病(MPD)的诊断价值。方法选择80例MPD患者,均采用骨髓活检-抽吸双样本同步取材,抽吸的髓液常规制成涂片,活检样本制成切片;分析骨髓涂片和骨髓切片的巨核细胞及巨核细胞簇形态特征及检出率。结果骨髓涂片与骨髓切片对各类型巨核细胞检出率比较差异均无统计学意义;骨髓涂片对A型检出率显著高于B型、C型(P<0.05),骨髓切片对C型检出率显著高于A型、B型(P<0.05)。结论骨髓涂片适于观察形态学改变,骨髓切片适于观察结构改变,将两者综合分析,能够为MPD的诊断提供更加准确的信息。
Objective To explore the diagnostic value of bone marrow smears for chronic myeloproliferative diseases.Methods A total of 80 cases of chronic myeloproliferative disease( MPD) patients were selected while double samples of marrow biopsy and aspiration of all the patients were taken. The cerebrospinal fluid samples of aspiration were made into smears and the biopsy samples were made into slices. The morphological characteristics and the detection rate of giant cells in the bone marrow smears and bone marrow biopsy were analyzed.Results The detection rate of bone marrow smears and bone marrow biopsy for various megakaryocytes was not significantly different. The detection rate of bone marrow smears for type A megakaryocytes was significantly higher than for type B and C( P〈0.05),while the detection rate of bone marrow biopsy for type C was significantly higher than for type A and B( P〈0.05). Conclusion The morphological changes are mainly observed with bone marrow smears,whereas structural changes are mainly observed with bone marrow biopsy. Comprehensive analysis of the two approaches can provide more accurate information for the diagnosis of MPD.
