摘要
目的探讨肝纤维化64层螺旋CT灌注成像评价指标及其与血清学指标的相关性。方法选取我院2015年10月-2016年11月收治的慢性乙型肝炎患者70例作为观察组,另选我院正常健康体检者19例作为对照组,收集患者各项临床资料,分析多层螺旋CT灌注成像评价指标及与血清学指标的相关性。结果在不同程度肝纤维化组间比较中,门静脉灌注量(PVP)、肝脏灌注指数(HPI)、血容量图(BV)、血流量图(BF)参数有明显差异(P<0.05);在各期肝纤维化中,PVP、BV、BF随肝纤维化等级的增加而减少,HPI随肝纤维化等级的增加而上升(P<0.05);PVP、BF、BV参数与HA、LN、ⅣC呈现负相关性(P<0.05);HPI参数与HA、LN、ⅣC呈现正相关性(P<0.05);结论螺旋CT灌注成像评价指标与血清学指标之间具有相关性,灌注成像参数在一定程度中可反映患者胶原沉积及微循环状态,联合检查可对患者血流灌注情况进行评估。
Objective To investigate the correlations between the evaluation indexes of 64-slice spiral CT perfusion imaging of liver fibrosis and serological parameters.Methods Seventy cases of chronic hepatitis B patients who were admitted to our hospital between October 2015 and November 2016 were selected as the observation group while nineteen cases of normal healthy subjects were selected as the control group.The clinical data was collected and the evaluation indexes of perfusion imaging and the correlations with serological parameters were analyzed. Results There was significant difference in PVP,HPI,BV and BF parameters between different levels of liver fibrosis（ P〈0.05）.PVP,BV and BF decreased with the increase of liver fibrosis grade,but HPI increased with the increase of liver fibrosis grade（ P〈0.05）.PVP,BF and BV were negatively correlated with HA,LN and IVC（ P〈0.05）. A positive correlation was detected between HPI parameters and HA,LN and IVC（ P〈0.05）.Conclusion A correlation is detected between the evaluation indexes of spiral CT perfusion imaging and serum markers.The perfusion imaging parameters can reflect the collagen deposition and microcirculation to some extent.The combined examination can be used to evaluate blood perfusion of patients.
