骨科手术患者切口感染发生情况及影响因素回顾性分析 被引量:15

摘要 目的分析骨科手术患者切口感染的发生情况,并探讨影响其切口感染的相关因素。方法采用系统性回顾法分析2009年1月-2016年5月于我院骨科住院部手术治疗的68例患者的临床资料,统计分析所有患者术后切口感染的发生情况,并通过非条件单因素和多因素logistic回归模型分析影响其切口感染的相关因素。结果 A组(感染组,n=18)患者平均年龄明显大于B组(非感染组,n=50),BMI较B组低且合并糖尿病例数较B组多,差异均具有统计学意义(P<0.05);A组急诊手术、Ⅲ、Ⅳ手术切口及腰部以下手术患者例数所占比例均明显高于B组,术中出血量多于B组,且手术时间及住院时间均较B组长,差异均具有统计学意义(P<0.05,P<0.01);年龄、糖尿病病史、手术类型、手术切口类型、手术部位及手术时间和住院天数为影响骨科手术患者切口感染的独立危险因素(P<0.05)。结论骨科手术患者术后切口发生感染率较高,其年龄、糖尿病病史、手术类型、手术切口类型、手术部位及手术时间和住院天数为影响骨科手术患者切口感染的独立危险因素。 Objective To analyze the incidence of incisional infections in recipients of orthopedic surgery and investigate the influencing factors of wound infections.Methods The clinical data of 68 patients who had undergone surgical treatment in our hospital between January 2009 and May 2016 was analyzed retrospectively.The incidence of incisional infections in all these surgical patients was analyzed statistically.Conditional univariate and multivariate logistic regression models were used to analyze the factors that caused wound infections.Results The average age of patients in Group A( incision infection group,n = 18) was far above that of Group B( non-incision infection group,n = 50).The BMI in Group A was lower than in Group B,and there were more patients with diabetes mellitus in Group A than in Group B.The difference was statistically significant( P〈0.05).The proportion of patients in Group A who underwent emergency surgery,Ⅲ,Ⅳ operation incision and below waist operations was significantly higher than that in Group B.There was more intraoperative blood loss in Group A than in Group B,and durations of surgery and hospital stay were longer than in Group B( P〈0.05,P〈0.01).The difference was also of statistical significance( P〈0.05).Age,history of diabetes,types of surgery,types of surgical incisions,sites of surgery,durations of surgery and length of hospital stay were the independent risk factors for incision infections in patients in the Department of Orthopedics( P〈0.05).Conclusion The rate of orthopedic surgery in patients with postoperative incision infections is high.Age,the history of diabetes,types of surgery,types of incisions,locations of surgery,durations of surgery and the length of hospital stay are independent risk factors that influence wound infections in recipients of orthopedic surgery.
