心源性肺水肿应用无创机械通气治疗效果研究

摘要 目的研究心源性肺水肿应用无创机械通气治疗的效果。方法从该院于2014年10月—2015年10月间收治的心源性肺水肿患者中,随机抽取58例作为此次实验的对象,以电脑随机的方式,将该次纳入的对象58例随机分组,每组患者病例数等同。应用有创机械通气方案为常规组患者进行治疗,应用无创机械通气方案为研究组患者进行治疗,观察无创机械通气治疗前后患者的二氧化碳分压、氧气分压、血氧饱和度、心率以及呼吸等状况,对比两组患者无创机械通气时间、临床症状缓解状况以及死亡率。结果研究组经过无创机械通气治疗后,二氧化碳分压(12.12±4.05)mmHg、氧气分压(90.11±12.99)mmHg、血氧饱和度(98.64±4.71)%、心率(80.64±12.77)次/min以及呼吸(20.55±4.67)次/min,各项指标显著改善,对比治疗前二氧化碳分压(55.41±20.34)mmHg、氧气分压(51.01±15.66)mmHg、血氧饱和度(80.11±10.77)%、心率(111.01±20.67)次/min以及呼吸(25.53±8.32)次/min,差异有统计学意义(P<0.05)。研究组死亡率10.34%、临床症状缓解时间(35.5±4.9)min以及无创机械通气时间(50.1±4.2)min,常规组27.58%、临床症状缓解时间(150.4±11.2)min以及无创机械通气时间(125.6±9.5)min,组间差异有统计学意义(P<0.05)。结论相比有创机械通气而言,无创机械通气治疗心源性肺水肿,疗效更加突出,可有效帮助患者改善临床症状,降低患者死亡率,缩短机械通气时间,临床价值高,值得推广。 Objective To research the effect of non-invasive mechanical ventilation in treatment of cardiac pulmonary edema. Methods58 cases of patients with cardiac pulmonary edema admitted and treated in our hospital from October 2014 to October 2015 were se-lected and randomly divided into two groups with 29 cases in each, the routine group and the research group respectively adopted the invasive mechanical ventilation and non-invasive mechanical ventilation for treatment, and the partial pressure of carbon dioxide,oxygen partial pressure, oxygen saturation of blood, heart rate and respiration conditions of patients before and after the non-invasive mechanical ventilation were observed and the time, clinical symptoms relief situation and death rate were compared between the two groups. Results After treatment, the various indexes of partial pressure of carbon dioxide, oxygen partial pressure, oxygen saturation of blood, heart rate and respiration in the research group were obviously improved, which were respectively(12.12±4.05)mmHg,(90.11±12.99)mmHg,(98.64±4.71)%,(80.64±12.77)times/min,(20.55±4.67)times/min, and these indexes before treatment were respectively(55.41±20.34)mmHg,(51.01±15.66)mmHg,(80.11±10.77)%,(111.01±20.67)times/min and(25.53±8.32)times/min, and the differences were statistically significant(P〈0.05), and the differences in the death rate, clinical symptoms relief time between the routine group and the research group were obvious[10.34%,(35.5±4.9)min,(50.1±4.2)min vs 27.58%,(150.4±11.2)min,(125.6±9.5)min](P〈0.05). Conclusion The curative effect of non-invasive mechanical ventilation in treatment of cardiac pulmonary edema is more ob-vious than that of invasive mechanical ventilation, which can effectively help improve the clinical symptoms of patients, reduce the death rate, shorten the mechanical ventilation time with high clinical value, and it is worth promotion.
