揉髌手法对兔膝骨关节炎模型软骨组织的影响 被引量:6

Effect of Kneading Manipulation on Cartilage Tissue in Rabbit Models with Knee Osteoarthritis

摘要 目的:观察揉髌手法对兔膝骨关节炎模型软骨组织的影响,为膝骨关节炎的防治提供实验依据。方法:将48只新西兰兔随机分为手法组、对照组、假模组、正常组,每组12只。手法组和对照组通过结扎下肢部分静脉造成骨内高压型膝骨关节炎模型;假模组行手术显露但不结扎切断相应血管;正常组则不做任何处理。自造模1周后手法组予揉髌手法,连续治疗5周,共17次,其余3组不予任何治疗措施。在造模后第12周末取右膝关节胫骨平台内侧软骨组织进行肉眼观察和切片光镜观察,并按照关节软骨改良Mankin分级评分标准进行病理学评分。结果:经软骨组织病理学Mankin评分,对照组评分与手法组、假模组及正常组比较明显较高,差异有统计学意义(P<0.05);手法组与假模组比较,差异无统计学意义(P=0.99>0.05)。对照组呈现中度退变,手法组及假模组为轻度退变,正常组未见明显退变。结论:揉髌手法能延缓软骨组织退变。 Objective:To observe the effect of kneading manipulation on the morphology of cartilage in rabbits with knee osteoarthritis,and to provide experimental basis for the prevention and treatment of knee osteoarthritis.Methods:Forty eight New Zealand rabbits were randomly divided into a manipulation group,a control group,a sham group and a normal group,with 12 rabbits in each group.In the manipulation group and the control group,the intraosseous hypertensive knee osteoarthritis models were induced by ligation of the lower part of the vein.The sham group underwent operation without ligation and cut-off of the corresponding blood vessels;and the normal group did not receive any treatment.One week after the models were made,rabbits in the manipulation group were treated with kneading manipulation for five consecutive weeks,a total of 17 times,and the remaining three groups were not given any treatment.At the end of the twelfth week after the establishment of the models,the medial articular cartilage of the right knee joint tibial plateau was taken out and observed with naked eyes and slice light microscope,and the pathological scores were evaluated according to the improved Mankin grading standard of the articular cartilage.Results:According to Mankin scores,the scores of the control group was significantly higher than those in the manipulation group,the sham group and the normal group,and the difference was statistically significant ( P 〈 0.05 ) .The difference between the manipulation group and the sham group was not statistically significant ( P = 0.99 〉 0.05 ) .The control group showed a metaphase degeneration,while the control group and the sham group showed a mild degeneration,and there was no obvious degeneration in the normal group.Conclusion:The kneading manipulation can delay the degeneration of cartilage tissue
