摘要
目的研究高同型半胱氨酸(Hcy)血症与叶酸、维生素B12水平的关系。方法将2015年9月至2016年9月在保山市人民医院就诊的109例高Hcy血症门诊和住院患者作为观察组,以同期65例Hcy正常的体检者作为对照组,比较两组血清Hcy、叶酸、维生素B12水平的差异并进行相关性分析。结果对照组维生素B12和叶酸水平高于高Hcy组[维生素B12(ng/L):563.7±303.8比345.9±220.1,叶酸(μg/L):11.5±4.5比8.1±4.50,均P<0.01],观察组Hcy与叶酸、维生素B12水平呈负相关(r_1=-0.527,r_2=-0.562,均P<0.01),对照组Hcy与叶酸、维生素B12水平相关性不显著(均P>0.05)。分析高Hcy组的Hcy、叶酸、维生素B12发现,维生素B12正常而叶酸不足或缺乏均能引起Hcy升高,但升高幅度没有维生素B12减低组患者(即使叶酸正常)Hcy升高的幅度大。结论在Hcy代谢过程中维生素B12和叶酸缺一不可;维生素B12可以提高叶酸利用率,维生素B12缺乏导致叶酸变成不能利用的形式,可能也是诱发高Hcy血症的重要因素之一,所以在高Hcy血症时动态监测血清叶酸、维生素B12水平对指导临床用药等方面应该具有重要的临床意义。
Objective To study the relationship between homocysteine (Hcy) and folic acid and vitamin B12 levels. Methods From September 2015 to September 2016, 109 cases of patients with high Hey in Baoshan People's Hospital were selected as the observation group, 65 cases with normal physical examination were used as control group. The serum levels of Hey, folic acid and vitamin B12 were retrospectively analyzed in two groups. Correlation analysis between Hcy and folio acid, vitamin B12 was performed. Results Vitamin B12 and folic acid levels in the control group were higher than those of vitamin BI2 and folie acid levels [vitamin B12 (ng/L): 563.7±303.8 vs. 345.9±220.1, folic acid (μg/L): 11.5±4.5 vs. 8.1 ±4.50, both P 〈 0.01), Hcy and folic acid, vitamin B12 levels were negatively correlated (r1 = -0.527, r2 = -0.562, P 〈 0.01)] in the control group, while there were no significant difference for Hey and folie acid, vitamin B12 levels in control group (P 〉 0.05). Hey, folie acid, vitamin B12 in the high Hey group were found that lack of normal folie acid vitamin B12 can cause Hey elevation, but no vitamin B12 reduction of patients (even iffolie acid normal) Hey increased significantly. Conclusions In the process of Hey metabolism, vitamin B12 and folie acid are indispensable; vitamin B12 may improve the utilization of folie acid, vitamin B12 deficiency led to the uselessness of folie acid, and may also be one of the important factors to induce high Hey, so the dynamic monitoring of serum folie acid, vitamin B12 levels in the high Hey blood disease should have important clinical significance to guide clinical medication and so on.
