摘要
目的了解男男性行为者(MSM)异地性行为现状,分析影响因素。方法 2016年2—11月,以四川省绵阳市辖区MSM为对象,采用滚雪球抽样法,在知情同意下,进行调查对象招募调查及血清学检测。结果合格调查1 073人中,有异地性行为者为6.7%。异地性行为地方为附近县的15.3%、绵阳34.7%、成都15.3%、省外等其他地区34.7%。每天平均性行为(1.6±1.038)次,偶然性伴72.1%、固定性伴27.9%。寻找性伴场所类型69.1%为网络型、11.8%为家庭型,肛交有保护性73.5%。异地性行为HIV感染率12.5%,高于无异地性行为的5.5%(P<0.05)。多因素分析,离异丧偶、本地居住时间短、生理需求为第一需求、最近1次肛交无保护性行为、近6个月肛交人数≥2发生异地性行为可能性较高(P均<0.05)。结论 MSM异地性行为以大中城市、网络型、偶然性伴为主,无保护、多性伴比例较高,增大了HIV感染和跨区域传播风险。
Objective To investigate the current status of off-site sexual behavior of men who have sex with men（MSM）,and analyze the influencing factors. Methods From February to November, 2016, the MSM of Mianyang City, SichuanProvince were used as the objects, and the survey was conducted with the snowball sampling method. Under the informedconsent, the survey including serological tests was conducted. Results Among 1 073 persons surveyed, the off-site sexualbehavior accounted for 6.7%. About the sites, the nearby county accounted for 15.3%, Mianyang City accounted for 34.7%,Chengdu City accounted for 15.3%, and other provinces accounted for 34.7%. The average of daily sexual behavior was（1.6 ±1.038） times, the occasional sexual partner accounted for 72.1%, and the fixed sex partner accounted for 27.9%. About how tofind the sexual partners, there were 69.1% of network type and 11.8% of family type. The protective anal sex accounted for73.5%. The rate of HIV infection was 12.5% in MSM who had off-site sexual behavior, which was higher than that in MSM whohad no off-site sexual behavior（5.5%）（P0.05）. The multivariate analysis showed that the divorced or widowed, short localresidence, physiological demand as the first, unprotected anal sex during the recent one, and anal sex persons ≥2 during therecent 6 months were more likely to have the off-site sexual behavior（P0.05）. Conclusions For off-site sexual behavior ofMSM, large and medium-sized cities, network type and accidental partner were the main place and type, and the unprotectedsexual behavior and multiple partners account for high proportions, which increases the risk of HIV infection and cross-regional transmission.
