摘要
目的检测RF、CCP与CIC-C1q在类风湿性关节炎(rheumatoid arthritis,RA)、其他关节炎与正常志愿者血清中的水平,比较循环免疫复合物(Circulating immune complex,CIC)与RF、CCP在RA中的检测能力,进而评估CIC在RA中的诊断价值。方法临床上收集RA患者、其他关节炎患者和正常志愿者各60人的血清,通过胶乳免疫增强法、间接酶联免疫吸附试验(ELISA)和C1q固相ELISA法分别检测血清中三个指标(RF、CCP、CIC-C1q)的水平,并进行统计学分析。结果 RF在RA患者组、其他关节炎患者组、正常志愿者三组中的浓度分别为:117.17±11.08IU/ml、21.42±4.66IU/ml、7.65±0.64IU/ml,CCP水平在三组中分别为:545.64±93.74AU/ml、13.21±3.32AU/ml、5.08±0.66AU/ml,CIC-C1q水平在三组中分别为:136.47±15.25ng/ml、89.67±13.85ng/ml、27.64±2.27ng/ml。CIC-C1q水平在三组间两两比较均存在统计学差异(P<0.0001),运用ROC曲线对三个指标进行分析得出在三个指标进行联合检测时特异度和灵敏度都最好。结论 CIC在RA的诊断中有一定的价值,将其与CCP、RF联合检测能提高RA诊断的灵敏度与特异度,并在RA与其他关节炎的鉴别诊断中也存在一定价值。
Objective To detect the level of serum RF,CCP and CIC-C1 q in rheumatoid arthritis(RA) patients,other arthritis patients and normal volunteers,the detection ability and diagnostic value of CIC in RA was compared and evaluated. Methods The clinical data of 60 patients with RA,other arthritis patients and normal volunteers were collected,respectively. The level of three indexes(RF,CCP and CIC-C1q) in serum were measured by latex immunoassay,indirect enzyme-linked immunosorbent assay(ELISA) and C1 q solid phase ELISA. All data was analyzed by statistical tools. Results The concentration of RF in three groups(RA,other arthritis patients and normal volunteers) were 117.17±11.08IU/ml,21.42±4.66IU/ml,7.65±0.64IU/ml,respectively. The level of CCP were 545.64±93.74AU/ml,13.21±3.32AU/ml,5.08±0.66AU/ml,respectively. And the level of CIC-C1 q were136.47±15.25ng/ml,89.67±13.85ng/ml,27.64 ±2.27ng/ml,respectively. There was significantly difference in the level of CIC-C1 q among three groups(P〈0.0001). The ROC curve showed that the specificity and sensitivity of these three indexes were best. Conclusion CIC has a certain value in the diagnosis of RA,and combined with CCP and RF can improve the sensitivity and specificity of RA diagnosis,with a great value in the differential diagnosis between RA and other arthritis.
