冠状动脉慢性完全闭塞病变患者红细胞分布宽度与冠状动脉侧支循环的关系

Relationship between red blood cell distribution width and coronary collateral circulation in patients with chronic total occlusion of coronary artery

摘要 目的 探讨冠状动脉慢性完全性闭塞(CTO)病变患者红细胞分布宽度(RDW)与冠状动脉侧支循环(CCC)的关系.方法 根据CCC形成情况将190例冠状动脉CTO患者分为CCC不良组110例和CCC良好组80例,抽取两组患者空腹静脉血检测RDW、Hb、中性粒细胞计数、淋巴细胞计数、尿素氮、血肌酐、总胆固醇、甘油三酯、尿酸水平并进行比较,采用Spearman相关分析及多元Logistic回归分析RDW和CCC的相关性.结果 两组患者之间性别、糖尿病病史、吸烟史、Hb、中性粒细胞计数、淋巴细胞计数、尿素氮、肌酐、尿酸、甘油三酯、总胆固醇比较,差异均无统计学意义(P>0.05).与CCC良好组比较,CCC不良组的年龄较大(P<0.05).CCC不良组有高血压病史患者比例低于CCC良好组(P<0.05).CCC不良组的RDW高于CCC良好组(P<0.05).Spearman相关分析结果显示,RDW与CCC分级呈负相关(r=-0.920,P <0.001).多元Logistic回归分析结果显示,RDW是CCC形成不良的独立预测因子.结论 在冠状动脉CTO病变患者中,高RDW与CCC生成不良密切相关,是CCC生成不良的独立预测因子. Objective To investigate relationship between the red blood cell distribution width (RDW) and coronary collateral circulation(CCC) in patients with chronic total occlusion (CTO) of coronary artery.Methods According to the formation of CCC,190 patients with CTO of coronary artery were divided into poor CCC group(110 cases) and of good CCC group(80 cases).Fasting blood samples were taken from patients of two groups to detect RDW,Hb,neutrophil count,lymphocyte count,urea nitrogen,serum creatinine,total cholesterol,triglyceride and uric acid.The correlation between RDW and CCC was analyzed by Spearman correlation analysis and multivariate Logistic regression analysis.Results There were no significant differences of gender,history of diabetes and smoking,Hb,neutrophil count,lymphocyte count,urea nitrogen,creatinine,uric acid,triglyceride and total cholesterol in the two groups(P 〉 0.05).Compared with good CCC group,age of poor CCC group was elder(P 〈 0.05).The proportion of patients with hypertension in poor CCC group was lower than that in good CCC group(P 〈0.05).RDW of poor CCC group was higher than that of good CCC group(P 〈 0.05).Spearman correlation analysis showed that RDW was negatively correlated with CCC grade (r =-0.920,P 〈 0.001).multivariate Logistic regression analysis showed that RDW was an independent predictor of poor CCC formation.Conclusion In patients with CTO of coronary artery,high RDW is closely related to poor CCC formation and is an independent predictor of it.
