摘要
目的探讨脂蛋白(a)和超敏C反应蛋白在急性脑梗死中的诊断价值研究。方法方便选取该院2015年1月—2017年2月期间来该院进行健康体检的30名健康人作为健康体检组,另外选取同期来该院收治的70例急性脑梗死患者作为急性脑梗死组。对比健康体检组的血清hs-CRP和Lp(a)水平与急性脑梗死组hs-CRP和Lp(a)水平,急性脑梗死组同NIHSS评分亚组间血清hs-CRP和Lp(a)水平进行比较,并对急性脑梗死组血清hs-CRP和Lp(a)间的关联性进行分析。结果急性脑梗死组的Lp(a)和hs-CRP水平分别为(333.72±32.56)mg/L和(5.32±2.60)mg/L,健康体检组两种检测项目血清水平分别为(146.30±21.17)mg/L与(0.87±0.44)mg/L,组间比较健康体检组明显低于急性脑梗死组,差异有统计学意义(t=29.411,4.573,P<0.05)。急性脑梗死组不同NIHSS评分亚组间血清Lp(a)[<5分:(284.43±25.79)mg/L;5~15分:(346.47±32.35)mg/L;﹥15分:(468.85±45.47)mg/L]和hs-CRP水平[<5分:(5.14±2.10)mg/L;5~15分:(8.63±2.45)mg/L;﹥15分:(11.52±3.18)mg/L]差异有统计学意义(P<0.05),随着评分的逐渐增高,检测血清指标水平呈现逐渐升高的趋势。相关性分析显示两者显著成正相关(r=0.684,P=0.011)。结论急性脑梗死的发生与发展过程血清Lp(a)和hs-CRP参与了以积极的干预措施,反映病情严重程度的重要生物学指标对疾病的发生、发展进行重要评估,降低急性脑梗死的死亡率,防治预后,具有重要的诊断价值。
Objective This paper tries to study the diagnostic value of lipoprotein(a) and hypersensitive C reactive protein in patients with acute cerebral infarction. Methods Convenient selection 30 healthy check-ups were selected as control group in this hospital from January 2015 to February 2017, and 70 cases of ACI patients in the same period were selected as the ACI group. The level of hs-CRP and Lp(a) of the two groups were compared. The correlation of Lp(a) and hs-CRP was analyzed The level of hs-CRP and Lp(a) of ACI patients and NIHSS group were compared. Results The level of hsCRP and Lp(a) of ACI group was(5.32±2.60)mg/L and(333.72±32.56)mg/L, and those of the healthy group was(0.87±0.44)mg/L and(146.30±21.17)mg/L, and the healthy group scores were obviously lower than the ACI group, the difference was significant(t=29.411, 4.573, P0.05); the level of Lp(a) of NIHSS points[5 points:(284.43±25.79)mg/L; 5~15 points:(346.47±32.35)mg/L;15 points:(468.85±45.47)mg/L] and hs-CRP [5 points:(5.14±2.10)mg/L; 5~15 points:(8.63±2.45)mg/L;15points:(11.52±3.18)mg/L], the difference was significant(P0.05); With the increasing of the points, the serum was gradually increased; the detection indicators were positively correlated with each other according to the correlation analysis(r=0.684,P=0.011). Conclusion The level of hs-CRP and Lp(a) is involved in the genesis and progress of acute cerebral infarction; it is considered as the important biological indicators on the genesis and progress of acute cerebral infarction. The positive nursing interventions can reduce the death rate of acute cerebral infarction and improve the favorable prognosis.
