摘要
中医黄疸病在临床上非常常见,其病理因素以湿邪为主,形成的关键是湿邪为患。黄疸的治疗主要为化湿邪,利小便,使湿邪从二便而出,以达到湿去黄退的目的。既往黄疸的理论认为,脏腑多涉及肝胆、脾胃,未涉及肺脏。肺主气,司呼吸,宣发肃降,通调水道,在水液代谢方面发挥着重要作用,同时肺和肝脏、脾胃联系紧密。文章根据中医脏腑相关理论,从肺与脾、肺与肝、肺与大肠的关系中,探讨宣调肺气,畅达中焦,祛脾之湿;佐金平木,肃肺清肝,清肝之热;金水相生,滋补肝肾,补肝之阴;肃肺清肠,利胆退黄,祛大肠之湿;宣肺解表,清利湿邪,解黄疸之表等方法,从肺论治黄疸,丰富中医黄疸病的治疗方法,以期有益于临床。
Jaundice in TCM is a very common disease in clinic. It is caused mainly by pathogenic damp in pathology and pathogenesis. The treatment of jaundice focuses on eliminating damp from benefiting urination. Theoretically,jaundice is analyzed as the dysfunction of liver,gallbladder,spleen and stomach,without lung function in consideration. Lung dominates qi and respiration. It controls the dispersing and descending of qi and regulates water passage. Lung function plays the important role in water metabolism. Moreover,lung functions communicate closely with liver,spleen and stomach. In the paper,regarding the relevant theories of zangfu organs,in view of the relationship between the lung and the spleen,the lung and the liver and the lung and the large intestine,the treating principles for damp removal are discussed,such as regulating lung qi and qi in middle Jiao to remove spleen damp,harmonizing the mental and the wood to clear liver heat,nourishing liver and kidney yin based on the mutual promotion of the metal and water,dispersing lung qi and benefiting the gallbladder to removing damp from the large intestine,dispersing lung qi and removing the exterior syndrome and damp to treat the exterior syndrome of jaundice. The therapeutic method for jaundice based on lung functions enriches the treatment of the disease in clinical practice.
