摘要
经典方剂半夏泻心汤主治寒热错杂的痞证,是治疗慢性萎缩性胃炎(CAG)的代表方剂。通过检索半夏泻心汤治疗CAG的研究文献,发现其对于改善临床症状和胃镜疗效具有明显作用,不良反应相对较少,但总体研究质量较低,基础研究证据非常有限。鉴于此,未来研究应设计随机、双盲、安慰剂对照试验,重视结局评价和随访研究,研发半夏泻心汤治疗CAG医疗机构制剂或中药新药,运用网络药理学方法阐释中药复方的作用机制。临床实践方面,笔者认为"寒热错杂、气虚血瘀"为CAG的关键病机,治疗以辛开苦降、益气活血法为主,在方证对应的基础上可能会提高临床疗效。
Banxia xiexin tang is the classic formula for the treatment of the gastric fullness and distention differentiated as mixture of cold and heat and is the representative formula for the treatment of chronic atrophic gastritis（ CAG）. Through retrieving the literatures for the treatment of CAG with banxia xiexin tang,it is found that this formula has the apparent effects on relieving clinical symptoms and improving endoscopic effects,and has relatively less adverse reactions. But,the general research quality is low,the basic research evidences are very limited. Hence,in the future research,the randomized,double blind and placebo control trial should be designed. The outcome evaluation and follow-up study should be emphasized. The agent or new drug should be developed to promote the application of banxia xiexin tang in the treatment of CAG in medical institutions. Using the network pharmacological method,the effect mechanism of herbal compound should be explained. In clinical practice,the writers believe that ＂ the mixture of cold and heat,qi deficiency and blood stagnation＂ are the keys in the pathogenesis of CAG. The herbs of pungent and bitter tastes are used for opening and descending actions,benefiting qi and activating blood circulation are the main treatment principles. The correct application of herbal formula corresponding to the syndromes may improve the clinical therapeutic effects.
