摘要
目的对比甲状腺常见的良性病变、恶性病变的CT表现和病理对照,分析二者的CT表现征象,探讨二者的CT诊断及鉴别诊断要点。方法随机选取150例在2015年1月—2017年1月期间与该院诊治并且已行甲状腺CT平扫、增强检查,且经手术及病理确诊的病例,回顾性分析其CT表现特点,得出良恶性病变的CT诊断及鉴别诊断要点。并对患者进行病理诊断,分析经CT表现诊断的甲状腺疾病的相符率。结果所有入选进行该次研究的患者中,经CT表现诊断出的良性甲状腺病变患者有108例,恶性甲状腺病变的患者有42例,经过术后病理诊断,与CT表现诊断的结果,差异无统计学意义(P>0.05)。关于甲状腺良性病变和恶性病变的CT表现对比:从病变边界不清上对比,良性病变边界不清的有18例(16.7%),恶性病变边界不清的有23例(54.8%),两者差异有统计学意义(P<0.05),从钙化来看,良性病变发生钙化的机率(13.9%)也要比恶性病变(47.6%)低的多,差异有统计学意义(P<0.05),从囊性变发生的概率来看,良性病变(43.5%)高于恶性病变(14.2%),差异有统计学意义(P<0.05),从包膜的不完整性看,良性病变(4.6%)低于恶性病变(76.2%),差异有统计学意义(P<0.05),从颈部的淋巴结是否出现肿大看,良性病变低于恶性病变,差异有统计学意义(P<0.05)。结论对甲状腺病变患者进行CT图像的诊断,可以有效的判断患者所患疾病的良恶性,根据CT图像中,患者病变组织的边界不清、钙化、囊性变和包膜的不完整性以及有无颈部的淋巴结肿大等特点对甲状腺病变的良恶性进行鉴别及诊断。
Objective To compare the CT manifestations and pathology of common benign and malignant lesions of thyroid and analyze the CT manifestation signs and study the CT diagnosis and differential diagnosis key points. Methods150 cases of patients with thyroid CT plain scan and enhancement examination in our hospital from January 2015 to January 2017 were randomly selected and the CT manifestation features were retrospectively analyzed after the operation and pathology diagnosis thus concluding the CT diagnosis and differential diagnosis key points, and the coincidence rate of thyroid disease was analyzed. Results There were 108 cases of patients with benign thyroid lesion diagnosed by CT manifestations and 42 cases with malignant thyroid disease, and the difference in the result between the postoperative pathological diagnosis and CT manifestation diagnosis was not obvious without statistical significance(P>0.05), and the comparison of CT manifestations of benign and malignant lesions showed that there were 18 cases whose benign lesions boundary was unclear(16.7%)and 23 cases whose malignant lesion boundary was unclear(54.8%), and the difference was obvious with statistical significance(P<0.05), and the calcification probability of benign lesion was much lower than that of malignant lesion(13.9% vs 47.6%), and the difference was statistically significant(P<0.05), and the cystic degeneration occurrence probability of malignant lesions was higher than that of malignant lesions(4.6% vs76.2%), and the difference was statistically significant, and the occurrence of swelling of lymph gland of the malignant lesions was lower than that of malignant lesions, and the difference was statistically significant(P<0.05). Conclusion The CT imaging diagnosis of patients with thyroid lesions can effectively determine the malignant and benign diseases of patients, and we can diagnose the malignant and benign thyroid lesions according to the features of unclear boundary of lesion tissues, calcification, cystis degeneration, incomplete cap
