摘要
目的探讨切开复位内固定治疗C型Danis-Weber踝关节骨折患者的临床疗效及安全性。方法选取2013年6月至2015年11月我院收治的64例C型Danis-Weber踝关节骨折患者,依据建档顺序分组,各32例。对照组采用手法复位,研究组采用切开复位内固定,术后3~6个月进行随访。随访期间统计对比两组住院时间及骨折愈合时间、踝关节功能优良率、并发症发生率,分析入院时及末次随访期间两组生活质量评分(QOL)及踝关节功能评分(Mazur)变化情况。结果研究组住院时间长于对照组,骨折愈合时间少于对照组,差异有统计学意义(P<0.05);治疗前两组QOL评分及Mazur评分比较,差异无统计学意义(P>0.05),治疗后研究组QOL评分及Mazur评分高于对照组,差异有统计学意义(P<0.05);研究组踝关节功能优良率90.63%(29/32)高于对照组65.63%(21/32),差异有统计学意义(P<0.05);研究组并发症发生率6.25%(2/32)低于对照组28.13%(9/32),差异有统计学意义(P<0.05)。结论采用切开复位内固定治疗C型Danis-Weber踝关节骨折效果显著,可有效减少骨折愈合时间,改善患者踝关节功能及生活质量,且安全性较高。
Objective To investigate the clinical efficacy and safety of open reduction and internal fixation for C-type Danis-Weber ankle fracture.Methods Sixty-four patients with type 6 Danis-Weber ankle fractures were enrolled in our hospital from June 2013 to November 2015,and32 patients were divided according to the order of archiving.The control group was reset by hand,and the study group was treated with open reduction and internal fixation.The patients were followed up for 3to 6 months.During the follow-up period,the hospitalization time and the time of fracture healing,the excellent rate of ankle joint function and the incidence of complications were analyzed.The changes of QOL and ankle function scores were analyzed at the time of admission and the last follow-up(P〈0.05).There was no significant difference between the two groups before and after treatment(P〉0.05).There was no significant difference between the two groups before and after treatment(P〉0.05).There was no significant difference between the two groups(P〈0.05).The excellent and good rate of ankle joint function was 90.63%(29/32)in the study group and 65.63%(21/32)in the control group,the difference was statistically significant(P〈0.05).The incidence of complications in the study group was 6.25%(2/32)lower than that in the control group(28.13%,9/32),the difference was statistically significant(P〈0.05).Conclusion The use of open reduction and internal fixation for the treatment of type 6 Danis-Weber ankle fracture is effective,which can effectively reduce the time of fracture healing,improve the function of ankle joint and quality of life,and the safety is high.
