二维斑点追踪显像定量评价风湿性二尖瓣狭窄患者左心室收缩功能改变

Quantitative Evaluation on the Postoperative Change of Left Ventricle Function in Rheumatic Mitral Valve Stenosis Patients by Two-Dimensional Speckle Tracking Echocardiography

摘要 目的应用二维斑点追踪显像(2D-STI)技术定量评价风湿性二尖瓣狭窄(RMS)患者左心室收缩功能的改变。方法选取55例RMS患者为RMS组,42例健康人为对照组,分别行常规超声心动图检查并记录左心房收缩末期内径(LAESD)、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)。应用2D-STI技术获得心尖位长轴切面整体纵向应变值及左心室各短轴切面整体圆周应变值作为评估指标,定量评价RMS患者左心室功能的变化。结果 (1)常规超声心动图结果比较:RMS患者LAESD较对照组增大,LVEF值较对照组减小,差异有统计学意义(P<0.05);RMS患者LVEDD、LVESD与对照组比较,差异无统计学意义(P>0.05);(2)2D-STI技术结果比较:RMS患者心尖四腔观、长轴观、二腔切面观收缩期纵向峰值应变值及二尖瓣环、乳头肌、心尖水平左心室短轴收缩期圆周峰值应变值均明显小于对照组(P<0.01)。结论 RMS患者左心室整体纵向应变值及圆周应变较正常对照组减小,表明RMS会导致左心室收缩功能的下降。 Objective To study the quantitative evaluation on the postoperative change of left ventricle function in rheumatic mitral valve stenosis patients by two-dimensional speckle tracking echocardiography.Methods Fifty-five identified patients with rheumatic mitral stenosis were enrolled in RMS group and forty-two healthy individuals formed control group.All subjects were examined by traditional echocardiography,and their left atrial end-systolic diameters, left ventricular end-diastolic diameters, left ventricular endsystolic diameters, left ventricular ejection fraction were recorded.2D-STI techniques were used to obtain the overall longitudinal strain of the apex and the total circumferential strain of the left ventricle as the evaluation index. The changes of left ventricular function in RMS patients were quantitatively evaluated. Results ① Comparing results measured by traditional echocardiography:compared with control group, left atrial end-systolic diameters in RMS group increased, left ventricular ejection fraction were lower, and differences had statistical importance (P〈0.05) ; left ventricular end-diastolic diameters, left ventricular end-systolic diameters in RMS had no statistical importance(P〉0.05). ②Comparing results of 2D-STI:In the RMS patients,global longitudinal strain in apical four-chamber view,apical long axis view,two-cavity sectional view and circumferential strains of mitral ring, papillary muscles,changes of left ventricle total circumferential strain in short axis at apex cordis level were obviously lower than control group (P〈0.01).Conclusion Global longitudinal and circumferential strains of patients' left ventricle in RMS group were lower than control group, showing that RMS can lead to reduce left ventricular systolic function.
