摘要
目的:比较CT增强扫描检查留置针拔除后手指按压法和棉球胶布加压按压法的效果。方法:选取2016年12月于我院行CT增强检查中意识清楚、能正确表达、愿意配合、无凝血功能障碍、一次穿刺成功且检查中无对比剂渗漏的1 465例患者,将其随机分为实验组(n=752)和对照组(n=713)。留置针拔除后,实验组采用棉球胶布加压按压法,而对照组采用传统的手指按压法。采用卡方检验比较2种按压法在患者满意度、疼痛等级及出血相关并发症的差异。结果:2组患者年龄、沟通能力、配合程度、凝血功能、血管穿刺难易程度等方面无明显差异。统计分析发现,与手指按压法相比,棉球胶布加压按压法可以提升患者满意度(97.03%vs.90.87,P=0.000),有效降低并发症发生率,中度及以上疼痛发生率(8.37%vs.18.65%,χ~2=33.348,P=0.000),针眼出血发生率(0.93%vs.2.67%,χ~2=6.312,P=0.012),血肿发生率(0.53%vs.1.96%,χ~2=6.181,P=0.013),皮下瘀斑发生率(0.80%vs.2.52%,χ~2=6.772,P=0.009)。结论:CT增强检查留置针拔除后采用棉球胶布加压按压法比手指按压法安全有效、预防按压并发症,而且简单易行,可以提高患者满意度,值得在临床应用中推广。
Objective:To compare the efficacy of finger pressing method and cotton ball with adhesive plaster pressing method after removal of the needle in CT enhancement scan. Methods:Totally 1 465 patients with clear consciousness,correct expression,willingness to cooperate and normal coagulation function received the CT enhancement scan in our hospital in December 2016,and all were,punctured for the first time without leakage of the contrast agent. The patients were randomly allocated into experimental group(n=752)and control group(n=713). Patients received a cotton ball with adhesive plaster pressing method in experimental group and finger pressing method for local compression after removal of the needle in CT enhancement scan. Chi-square test was used to exam-ine the difference of patient satisfaction,pain degree,and bleeding associated complications. Results:Patients’ age,communication ability,cooperative degree,coagulation function,and difficulty degree of vascular puncture were comparable between the two groups.Compared with finger pressing method,cotton ball with adhesive plaster pressing method was associated with improved satisfaction rate(97.03% vs. 90.87,P=0.000)and decreased risks of complications,including incidence of moderate and above pain(8.37% vs.18.65% ,χ~2=33.348,P=0.000),incidence of needle eye bleeding(0.93% vs. 2.67% ,χ~2=6.312,P=0.012),incidence of hematoma(0.53% vs. 1.96% ,χ~2=6.181,P=0.013),and incidence of subcutaneous hemorrhage(0.80% vs. 2.52% ,χ~2=6.772,P=0.009). Conclusion:The cotton ball pressure press method is more safe,effective,easy and better prevent compression complications and improve patient satisfaction than finger press method. So,it should be promoted to clinical applications.
