摘要
目的研究针对进展期远端胃癌患者提供腹腔镜下D2根治术治疗的效果。方法该次研究方便选取该院于2012年8月—2014年3月期间收治的86例远端胃癌患者,按照随机数字表法将患者划分为对照组与观察组,每组43例,科室为对照组患者采用常规开腹胃癌D2根治术治疗,为观察组患者提供腹腔镜下胃癌D2根治术治疗,观察两组患者手术指标情况、并发症情况;随访患者3年观察生存率及复发率情况。结果对照组患者手术用时(183.29±15.38)min、术中出血为(332.29±75.21)m L、离床活动为(4.51±1.02)d、住院时间为(17.49±3.31)d、腹部切口为(16.39±1.24)cm;观察组手术用时(159.35±13.02)min、术中出血(151.92±28.10)m L、离床活动(3.01±0.82)d、住院时间(12.73±3.29)d、腹部切口(6.02±0.51)cm(P<0.05);对照组患者手术用时、术中出血、离床活动、住院时间、腹部切口等指标结果均高于观察组(P<0.05)。对照组患者术后并发症发生率为11例(25.58%),观察组为4例(9.30%),观察组明显低于对照组(P<0.05)。结论该院为胃癌患者提供腹腔镜下胃癌D2根治术治疗,取得疗效明显,并发症发生率较低,且术后恢复用时较短,腹腔镜手术与开腹手术治疗后,三年生存率及复发率结果相似,腹腔镜胃癌D2根治术值得临床应用及推广。
Objective To research the effect of laparoscopic D2 total radical gastrectomy in treatment of local advanced distal gastric cancer. Methods Convenient selection 86 cases of patients with distal gastric cancer admitted and treated in our hospital from August 2012 to March 2014 were selected and randomly divided into two groups with 43 cases in each, the control group and the observation group were respectively treated with routine laparotomy gastric cancer D2 total radical gastrectomy and laparoscopic D2 total radical gastrectomy, and the operation indexes and complications were observed and the 3-year survival rate and recurrence rate of patients were followed up. Results The operation time, intraoperative bleeding, time to leave bed, length of stay and abdominal surgical wound in the control group were higher than those in the observation group[(183.29±15.38)min,(332.29±75.21)m L,(4.51±1.02)d,(17.49±3.31)d,(16.39±1.24)cm vs(159.35±13.02)min,(151.92±28.10)ml,(3.01±0.82)d,(12.73±3.29)d,(6.02±0.51)cm](P<0.05), and the incidence rate of complications after operation in the observation group was obviously lower than that in the control group, [4 cases vs 11 cases, 9.30% vs 25.58%](P<0.05). Conclusion The curative effect of laparoscopic D2 total radical gastrectomy in treatment of local advanced distal gastric cancer is obvious with lower incidence rate of complications and short recovery time after surgery and the 3-year survival rate and recurrence rate are similar, which is worth application and promotion.
