280例重症肺炎患者病原菌分布及细菌耐药性分析

摘要 目的 研究280例重症肺炎患者病原菌的分布及细菌耐药性.方法 选择2014年3月-2016年4月在我院EICU住院的280例重症肺炎患者为研究对象,均收集痰标本并进行药物敏感性实验.结果 共检出327株病原菌,其中革兰阳性菌占30.89%、革兰阴性菌占59.94%、真菌占9.17%.革兰阴性菌以肺炎克雷伯菌最常见,革兰阳性菌以金黄色葡萄球菌最常见,真菌以克柔假丝酵母菌常见.亚胺培南、美罗培南对革兰阴性菌敏感度较高,万古霉素对革兰阳性菌敏感度较高.结论 革兰阴性菌在重症肺炎的病原菌中最为常见,各细菌对抗菌药耐药性较高,临床应对细菌耐药性变化实施检测,根据其变化选择合理的抗菌药. Objective To study pathogenic bacteria distribution and bacterial drug resistance in 280 patients with severe pneumonia. Methods 280 patients with severe pneumonia admitted in EICU of our hospital from Mar. 2014 to Apr. 2016 were selected. Sputum samples were collected and drug sensitivity assay was conducted. Results 327 cases of pathogenic bacteria were detected,including 30.89% of gram positive bacteria,59.94% of gram negative bacteria and 9.17% of fungus. Klebsiella pneumoniae was the most common in gram negative bacteria. Staphylococcus aureus was the most common in gram positive bacteria. Candida krusei was the most common in fungus. Imipenem and meropenem had higher sensitivity to gram negative bacteria. Vancomycin had higher sensitivity to gram positive bacte- ria. Conclusion The gram negative bacteria is the most common in pathogenic bacteria of severe pneumonia. Bacteria has high drug resistance to antibacterial drugs. Changes of bacterial drug resistance need to be detected so as to select reasonable antibacterial drugs.
