摘要
目的探讨孕中期唐氏征筛查对高龄妊娠孕妇是否具有预测意义。方法回顾该院2016年行介入性产前诊断的高龄孕妇296例,追溯其孕中期唐氏征筛查情况,结合羊水穿刺结果,比较其唐氏综合征的发生率;另外回顾该院2016年所有唐氏征筛查孕妇,分为高龄孕妇组和育龄孕妇组,比较两组孕妇中唐氏综合征的检出率。结果在296例高龄孕妇,唐氏征高风险人群62人,唐氏综合征发生率6.5%(4/62),而未行唐氏征筛查,仅以高龄为产前诊断指征的高龄孕妇194人,唐氏综合征的发生率为1.0%(2/194);在唐氏征筛查中,高龄孕妇提示高风险人数为282人,其中5个属染色体异常,染色体异常检出率:5/282(1.77%);育龄孕妇中提示唐氏征筛查高风险人数为655人,其中2个属于染色体异常,染色体异常检出率:2/655(0.3%)。结论孕中期唐氏征筛查法对高龄妊娠仍具有重要临床实用价值。
Objective:To explore the meaning of Down′s syndrome screening in the middle period of pregnant for women of advanced maternal age. Methods:Review the patients who had performed amniocentesis In 296 pregnant women of advanced maternal age because of advanced age and the high‐risk of Down′s screening at 2016. Review the results of their Down′s syndrome screening in the middle period of pregnant,combined with the results of prenatal diagnostic. Compared the incidence of the Down′s syndrome;In addition,review the patients who had done the prenatal screening of the Down′s syndrome,divided into advanced maternal age and reproductive age,Compared the detection ratio of the Down′s syndrome. Results:62 pregnant women with an increased risk of D S ware found in 296 pregnant women of advanced maternal age,the incidence of Trisomy 21 was 6.5%(4/62),76 pregnant women without prenatal screening were found in 201 pregnant women of advanced maternal age,the incidence of Trisomy 21 was 1.0%(2/194). 5 abnormal chromosome-al karyotypes were found in 282 pregnant women of advanced maternal age with an increased risk of D S,the detection ratio of the Chromosome abnormality was 5/282(1.77%);2abnormal chromosomal karyotypes were found in 655 pregnant women of reproductive maternal age with an increased risk of D S,the detection ratio of the Chromosome abnormality was 2/655(0.3%). Conclusion:It is still value to do the Down′s syndrome screening in the middle period of pregnant for women of advanced maternal age.
