摘要
目的探讨剖宫产手术时机选择不当、剖宫产史的再次妊娠剖宫产和手术技能缺陷对孕妇分娩结局的不良影响和预防。方法对2004年3月—2017年3月在广州市白云区太和镇卫生院(广州市白云区龙归华侨医院)发生的剖宫产副损伤的病例进行回顾性分析。结果 15例副损伤中,剖宫产时机选择不当5例,最佳手术时机延误先露下降过低,术中娩出胎头困难,导致宫颈纵行贯通性裂伤3例,其中1例缝合时尿管损伤,2例子宫下段切口延裂,均为产后出血。膀胱损伤2例,均为第3次剖宫产。剖宫产手术技能缺陷导致裂伤和血肿8例。结论剖宫产手术时机选择不当,有剖宫产史的再次剖宫产术,手术技能不成熟是导致副损伤的重要原因。所以加强产程管理早期识别难产并及时处理,降低首次妊娠剖宫产率,提高剖宫产手术技能是减少剖宫产副损伤的重要因素。
Objective To investigate the adverse effects and prevention of the timing of cesarean section, the choice of cesarean section, re pregnancy, cesarean section and surgical skills defects in pregnant women. MethodsA retrospective analysis was made on the side effects of caesarean section in taihe hospital of baiyun district（guangzhou baiyun district, longgui overseas Chinese hospital） from March 2004 to March 2017. Results 15 cases of injury, time of cesarean section in 5 cases of improper selection, optimal operation timing delays first dew drops too low, difficult to take the first surgery, cause cervical transfixion longitudinal laceration in 3 cases, including 1 case of catheter suture injury, 2 cases of lower uterine segment dehiscence, for postpartum hemorrhage. There were 2 cases with bladder injury, all of whom had third caesarean section. Defects in cesarean section resulted in laceration and hematoma in 8 cases. Conclusion The choice of timing of cesarean section is improper, the caesarean section with caesarean section history, the immaturity of surgical skills is an important cause of side injuries. Therefore, it is an important factor to decrease the cesarean section rate and improve the skills of cesarean section for reducing the rate of cesarean section and improving the skills of cesarean section.
