丙泊酚复合瑞芬太尼麻醉对腹腔镜卵巢囊肿切除术患者术后意识状态和认知功能的影响 被引量:9

摘要 目的:探讨丙泊酚复合瑞芬太尼麻醉对腹腔镜卵巢囊肿切除术患者术后意识状态和认知功能的影响。方法:选择2013年1月至2016年12月行腹腔镜卵巢囊肿切除术的患者120例,随机分为观察组和对照组,各60例;观察组采用丙泊酚复合瑞芬太尼麻醉,对照组采用静脉及吸入复合麻醉,观察两组麻醉效果。结果:麻醉后两组血压和心率较麻醉前均出现下降(P<0.05);两组在诱导后、插管时、气腹5 min后以及停药时收缩压、舒张压、心率等指标比较,差异无统计学意义(P>0.05);两组苏醒时间、定向力恢复时间比较,差异无统计学意义(P>0.05);观察组麻醉诱导时间低于对照组(P<0.05);观察组在拔管时、离开手术室时、拔管后1 h的警觉-镇静量表(the observers assessment of altertness/sedation scale,OAAS)评分等均高于对照组(P<0.05);两组术后1 h和术后3 h的简易智力状态检查(ninimental state examinatlon,MMSE)评分比较,差异无统计学意义(P>0.05)。结论:丙泊酚复合瑞芬太尼麻醉不会影响患者的呼吸、循环系统和认知功能,而且起效更快,苏醒质量更高,麻醉效果满意。 Objective: To investigate the effect of propofol combined with remifentanil anesthesia on postoperative conscious state and cognitive function in patients after laparoscopic ovarian cystectomy. Methods: 120 patients hospitalized from January 2013 to December 2016 after laparoscopic ovarian cystectomy were chosen and randomly divided into the observation group and the control group,60 cases in each group. The observation group was narcotized by propofol remifentanil while the control group was narcotized by inhalation anesthesia,with the effect of anesthesia observed. Results: Blood pressure and heart rate after anesthesia in the two groups decreased significantly( P〈0. 05); there was no difference in systolic pressure,diastolic pressure and heart rate after induction and discontinuation of medicine,during intubation and 5 min after pneumoperitoneum between the two groups( P〈0. 05); there was no difference in recovery time,time for orientation recovery between the two groups( P〉0. 05); the induced anesthesia time in the observation group was significantly lower than that in the control group( P〈0. 05). The OAAS scores during extubation,on leaving the operation room and 1h after extubation in the observation group were higher than those in the control group( P〈0. 05); there was no difference in MMSE scores 1h and 3h after the surgery( P〉0. 05). Conclusion: Propofol combined with remifentanil anesthesia does not affect respiration,circulatory system and cognitive function of patients. It takes effect faster,has higher recovery quality and better anesthesia satisfaction.
