多层螺旋CT薄层扫描与后处理技术对鼻骨骨折的诊断价值 被引量:8

摘要 目的探讨多层螺旋CT薄层扫描结合多平面重建(MPR)及容积再现(VR)后处理技术对鼻骨骨折的诊断价值。方法对62例来我院就诊的鼻部外伤患者行64层螺旋CT薄层扫描,每位患者诊断过程如下:首先在影像归档和通信系统(PACS)上对扫描的薄层轴位图像进行连续层面观察,作出初步诊断,然后在工作站上行MPR及VR重建,多方位多角度动态观察,得出最终诊断。结果初步诊断42例鼻骨骨折,其中多发骨折28例,单发骨折14例。28例多发骨折最终诊断有25例相符,3例过度诊断,分别将鼻骨孔、鼻骨表面压迹、鼻骨下端驼峰样变异误诊为骨折;14例单发骨折中有8例相符,6例不符,其中3例应为多发骨折,另外3例分别将鼻骨孔、缝间骨、鼻骨下端缺损误诊为骨折。初步印象8例未明确诊断,最终均作出了明确诊断:3例为骨折,2例为鼻骨下缘缺损变异,1例为鼻骨驼峰样变异,2例为鼻骨孔;初步诊断12例未见异常,最终有10例符合,2例为骨折。结论多层螺旋CT薄层扫描结合MPR及VR后处理技术对诊断鼻骨骨折具有重要价值。 Objective To explore the diagnostic value of multi-slice spiral CT and MPR,VR post-processing techniques in nasal bone fracture. Methods Sixty-two patients with nasal trauma were scanned by 64-spice spiral CT, all patients were diagnosed by the following process : at first, the thin layer axis images of the scanning were ob- served on the PACS continuously, made a preliminary diagnosis, then multi-direction and multi angle dynamic obser- vation between multi-plane reconstruction (MPR) and volume rendering(VR)were carried out in the workstation, drew a final diagnosis. Results Forty-two patients with nasal bone fracture were diagnosed at first, 28 patients with multiple fractures, 14 patients of single fracture. Of 28 patients, only 25 patients were consistent finally, there were over-diag- nosis of 3 patients, 1 was Foramen of nasal bone, 1 was surface depression of nasal bone, 1 was hump sample varia- tion of nasal bone; 14 patients of single fracture, 8 patients were consistent, 6 patients were inconsistent, a final diag- nosis of the remaining were respectively multiple fractures (n=3), foramen of nasal bone (n=1), wormian bone (n=1), congenital defect variation of nasal bone(n=1); 8 patients were diagnosed as no explicit exception at first, finally all were made definite diagnosis: 3 were fractures, 2 were congenital defect variations of nasal bone, 1 was hump sample variation of nasal bone, 2 were foramen of nasal bone; 12 patients had no abnormal at first, 10 patients were consis- tent finally, 2 patients were fractures. Conclusion It is significant valuable in the diagnosis of nasal bone fracture by combining multi-slice spiral CT with MPR, VR in the post -processing techniques.
