摘要 目的:分析MRI与CT在强直性脊柱炎骶髂关节病变诊断中的应用效果。方法:选取2015年1月-2016年7月我院收治的60例强直性脊柱炎骶髂关节病变患者,所有患者分别进行MRI和CT检查,比较两种检查方法下影像学表现、病变程度及骨质异常检出率。结果:MRI检查Ⅰ级强直性脊柱炎骶髂关节病变敏感性为38.33%,高于CT检查的21.67%,差异显著(P<0.05),Ⅱ级以上病变,两种检查方法敏感性比较无显著差异(P>0.05);骨质侵蚀及骨质硬化中,CT检出率分别为71.67%、86.67%,明显高于MRI检出率的35.00%、63.33%,差异显著(P<0.05);关节软骨肿胀检出率比较中,CT低于MRI,差异显著(P<0.05)。结论:MRI与CT均可对强直性脊柱炎骶髂关节病变进行诊断,在临床诊断中,可以将两种检查方式有效的结合,根据两者检查结果给出治疗方案,有助于提高临床检出率,为患者后期的治疗提供临床指导。 Objective: to analyze the effect of MRI and CT in the diagnosis of ankylosing spondylitis. Methods: January 2015-2016 year in July in our hospital 60 cases of ankylosing spondylitis sacroiliac joint disease patients,all patients underwent MRI and CT examination,comparison of two methods under the imaging findings,pathological changes and bone abnormality rate. Results: MRI examination of sacroiliac joint in ankylosing spondylitis sensitivity was 38. 33%,higher than the CT examination of 21. 67%,significant difference( P 〈 0. 05),above grade II lesions,there is no significant difference between the sensitivity of the two methods( P 〉 0. 05); bone erosion and bone sclerosis,CT detection rate was 71. 67%86. 67%,the detection rate of MRI was significantly higher than that of 35% and 63. 33%,significant difference( P 〈0. 05); the detection rate of swelling of articular cartilage in CT is lower than MRI,significant difference( P 〈 0. 05). Conclusion: MRI and CT can diagnose the sacroiliac joint in ankylosing spondylitis,in clinical diagnosis,can be combined with two kinds of inspection methods effectively,according to the inspection results given treatment,help to improve the clinical detection rate,and provide clinical guidance for the treatment of patients with late stage.
