Current situation and thinking of nosocomial infection management in military basic medical institutions

摘要 目的了解部队师旅医院、卫生队及团卫生队医院感染管理、消毒质量、发热门诊建设情况,为加强基层部队医院感染管理提供依据。方法 2016年5-6月采用问卷调查、现场查看和抽样检测等方法对20所师旅医院、卫生队及团卫生队医院感染管理情况的现状进行调查,医院感染控制的相关知识满分为100分,60分为合格;采用纸片法检测含氯消毒剂浓度;使用ATP荧光检测仪检测医务人员手卫生情况;使用紫外强度计测定使用中的紫外线灯;采用121℃压力蒸汽灭菌化学指示卡进行压力蒸汽灭菌器检测。结果调查的20所师旅医院、卫生队及团卫生队,仅3所师医院有医院感染控制工作人员,占15.00%;被调查的40名工作人员的医院感染基础知识合格率为40.00%;17所有独立的消毒室,占85.00%;师旅医院、卫生队及团卫生队使用中含氯消毒剂浓度均达到100.00%;紫外线灯辐照强度合格率,师旅医院、卫生队及团卫生队分别为95.00%和54.17%,差异有统计学意义(P<0.05);这20所基层部队医院或卫生队发热门诊感染防控设施完整率调查结果中,疫情报告制度完整率为100.00%;医务人员上岗前培训率较低,仅为15.00%。结论师旅医院、卫生队及团卫生队医院感染管理现状存在较多问题,是部队卫生工作的隐患,急需制定相关管理标准,进行人员培训、设施改善。 OBJECTIVE To understand the nosocomial infection management, disinfection quality, and fever outpatient building construction of division and brigade hospitals and health teams and regiment health teams, so as to provide the basis for strengthening the nosocomial infection management of grassroots units.METHODS From May to June in 2016, the status of nosocomial infection management in 20 division and brigade hospitals, health teams and regimental health teams were investigated by questionnaire, site inspection and sampling test.The knowledge of nosocomial infection control was divided into 100 points and 60 points were qualified.The concentration of chlorine disinfectant was detected by paper method.The hand hygienic condition of the medical personnel was measured by ATP fluorescence detector.The UV lamp was measured by UV intensity meter.Pressure steam sterilizer was tested at 121 ℃pressure steam sterilization chemical indicator.RESULTS Among the surveyed 20 division and brigade hospitals and health teams and regiment health teams, only three divisions hospitals had nosocomial infection control staff, accounting for 15%.The qualified rate the basic knowledge of the sense of nosocomial infection of the surveyed 40 staff was 40%.There were independent disinfection rooms in 17 units, accounting for 85%.The qualified rate of chlorine disinfectant concentration had all reached 100.00%.The qualified rate of UV lamp irradiation intensity in the division and brigade hospitals and health teams and regiment health teams were 95.00% and 54.17% respectively, and the difference was statistically significant (P&lt;0.05).In the survey of infection prevention and control facilities in fever clinic, the coverage rate of the epidemic reporting system was 100.00%, and the training rate of the medical staff was only 15.00%.CONCLUSION There are many problems in the current situation of nosocomial infection management in the division and brigade hospitals and health teams and regimental health teams, which urgently needs to f
