摘要
目的探讨联合检测血管内皮生长因子(VEGF)以及妊娠相关蛋白(PAPP-A)在先兆流产和异位妊娠鉴别诊断中的临床应用价值。方法选择宫内早孕患者60例(正常早孕组)、先兆流产患者50例(先兆流产组)以及异位妊娠患者40例(异位妊娠组),检测并比较三组血清VEGF及PAPP-A水平。结果与正常早孕组比较,异位妊娠组和先兆流产组血清VEGF表达水平均显著的增高,PAPP-A表达水平显著降低,差异有统计学意义(P<0.05);与先兆流产组比较,异位妊娠组患者血清VEGF表达水平显著增高,PAPP-A表达水平明显降低,差异有统计学意义(P<0.05)。联合检测VEGF与PAPP-A对异位妊娠的诊断敏感性均明显高于各单项检测,差异有统计学意义(P<0.05)。结论血清VEGF水平在异位妊娠和先兆流产患者中明显的升高,PAPP-A的水平明显的降低,联合检测两者有助于早期诊断和鉴别诊断先兆流产及异位妊娠。
Objective To investigate the clinical application value of combined detection of vascular endothelial growth factor(VEGF)and pregnancy associated protein A(PAPP-A)in the differential diagnosis of threatened abortion and ectopic pregnancy.Methods 60 patients with intrauterine pregnancy(normal early pregnancy group),50 patients with threatened abortion(threatened abortion group),40 patients with ectopic pregnancy(ectopic pregnancy group)were selected.The serum VEGF and PAPP-A level were detected.Results Compared with the normal early pregnancy group,serum VEGF level of ectopic pregnancy group and threatened abortion group were higher,serum PAPP-A level was lower,the differences was statistically significant(P〈0.05).Compared with the threatened abortion group,VEGF level of ectopic pregnancy group was higher,serum PAPP-A was lower than that of threatened abortion group,the differences was statistically significant(P〈0.05).The sensitivity and specificity of combined detection of VEGF and PAPP-A in the diagnosis of ectopic pregnancy were significantly higher than that of the single detection,the differences were statistically significant(P〈0.05).Conclusion The levels of serum VEGF is significantly increased in the patients with ectopic pregnancy and threatened abortion,and the PAPP-A level is significantly lower.Combined detection of both is helpful to early diagnosis and differential diagnosis of threatened abortion and ectopic pregnancy.
