摘要
目的探讨四维超声(4D)、断层超声显像(TUI)在膀胱肿瘤诊断中的应用价值。方法选取我院2013年9月至2015年5月收治的41例膀胱肿瘤患者的49个病灶,对其进行二维超声、4D及TUI检查,诊断结果与膀胱镜及病理诊断结果相比较,分析4D+TUI对膀胱肿瘤诊断的应用价值。结果膀胱镜检及病理诊断本组49个病灶中46个为膀胱肿瘤,恶性者占89%(41/46),良性者占11%(5/46),3个为膀胱内凝血块,并非真正的肿瘤。超声对膀胱占位性病灶诊断符合率二维超声84%(41/49)、4D+TUI 94%(46/49);膀胱肿瘤病理检查结果为良恶性肿瘤,超声诊断符合率4D+TUI 100%和94%明显优于二维超声诊断90%和81%。2组比较差异具有统计学意义(P<0.05)。结论 4D重建及TUI能够清晰显示肿瘤与膀胱壁的连接关系以及肿瘤的空间立体形态,获得全方位的多断层图像,提高膀胱肿瘤的检出率和诊断率。同时该技术方法简便、无损伤、价格低、可重复操作,患者易于接受,适用于临床推广应用。
Objective To study the value of 4Dand tomographic ultrasound imaging in bladder tumors.Methods Selected 49 focuses form 41 patients who had bladder tumors from our hospital from September 2013 to May 2015.We compare their diagnosis of 2D,4D＋TUI with cystoscope and pathology to analyse the value of 4D＋TUI.Results There are 46 bladder tumors＇ patients in 49 focuses.There are 89%（41/46）malignant patients,11（5/46）% benign persons and three patients who have non-bladder tumor,just have clot in bladder.The coin cidence of bladder space occupying＇s focuses is different between 2D and 4D＋TUI（2Dis 84%,4D＋TUI is 94%）.The coincidence of bladder tumors pathology is also different.（4D＋TUI is 100% and 94%）It is better than 2D.The differences have the meaning of statistics.Conclusion 4D and TUI can display the tumor connection relationship with the bladder wall and the spatial formation of tumor.The technology improved the detection rate and the diagnostic rate of bladder tumor.The method is easy to control,non-invasive,replicable,money-saving,and acceptable to patients.So it is suitable for clinical application.
