摘要
目的探讨输卵管性不孕患者生殖道病原菌的分布,为临床治疗提供参考依据。方法选择2014年1月-2015年12月医院收治的腹腔镜手术的输卵管性不孕患者300例为输卵管不孕组和妇科良性疾病腹腔镜手术的患者300例为对照组;对所有患者的阴道分泌物和盆腔积液中的病原菌进行检测。结果输卵管不孕组阴道清洁度Ⅲ~Ⅳ度患者占16.0%,高于对照组的4.0%(P<0.05);输卵管不孕组阴道分泌物菌群多样性和菌群密集度与对照组比较,差异有统计学意义(P<0.05);输卵管不孕组阴道表皮葡萄球菌、大肠埃希菌、支原体检出率均高于对照组,乳酸杆菌检出率低于对照组(P<0.05);输卵管不孕组盆腔积液乳酸杆菌、表皮葡萄球菌、大肠埃希菌、粪肠球菌、支原体的检出率均高于对照组(P<0.05)。结论输卵管性不孕患者阴道分泌物中优势菌群发生变化,盆腔积液中病原菌检出率升高,存在菌群失调。
OBJECTIVE To investigate the distribution of pathogens of reproductive tract in patients with tubal infertility,so as to provide references for clinical treatment.METHODS A total of 300 patients with tubal infertility underwent laparoscopic surgery were selected as tubal infertility group and 300 cases of gynecological benign disease laparoscopic surgery were set as control group from Jan.2014 to Dec.2015.The pathogenic microorganisms of vaginal discharge and pelvic fluid were detected in all patients.RESULTS The vaginal cleanlinessⅢ~Ⅳ degree of oviduct infertility group accounted for 6.0% ,which was higher than 4.0% of control group（P〈0.05）.There were significant differences in the bacterial diversity and the density of vaginal secretions between infertility group and control group（P〈0.05）.The detection rates of vaginal Staphylococcus epidermidis,Escherichia coli and mycoplasma of infertility group were higher than those of control group（P〈0.05）,and the detection rate of Lactobacillus was lower than that of control group（P〈0.05）.The detection rates of Lactobacillus,S.epidermidis,E.coli,Enterococcus faecalis and Mycoplasmain tubal infertility group were higher than those in control group（P〈0.05）.CONCLUSION The dominant flora of vaginal secretions in patients with tubal infertility have changed.The detection rate of pathogenic microorganisms of pelvic effusion increases,and flora imbalance exists.
