摘要
目的研究超声脂肪检测在非酒精性脂肪肝中的应用价值。方法选取2016年4月—2017年3月我院收治的非酒精性单纯性脂肪肝(NAFL)患者108例、非酒精性脂肪性肝炎(NASH)患者88例作为研究对象,全部患者均进行超声脂肪肝分级、Fibrotouch肝脏受控衰减参数(controlled attenuation parameter)检测、甘油三酯、总胆固醇以及脂肪特异性激素脂联素与内脂素等指标检测,比较分析两组患者检测结果。结果 NAFL组患者CAP、TG、TC、Acrp30水平低于NASH组,Visfatin水平高于NASH组,差异均具有统计学意义(P<0.05)。Pearson相关分析结果表明,NAFL组肝脏CAP值与TG、TC呈中等相关(P<0.05),与Acrp30、Visfatin呈强相关(P<0.05);NASH组肝脏CAP值与TG、TC呈中等相关(P<0.05),与Acrp30、Visfatin呈强相关(P<0.05);NAFL组、NASH组肝脏CAP与脂肪肝超声分级均呈极强相关(P<0.05)。结论非酒精性脂肪肝病应用CAP检测与彩色多普勒超声及血清脂肪相关指标一致性良好。
Objective To study the clinical values of ultrasonic fatty liver examination in patients with nonalcoholic fatty liver disease. Methods From April 2016 to June 2017, 108 cases of NAFL (non-alcoholic fatty liver) and 88 cases of NASH (nonalcoholic steatohepatitis) were recruited. CAP (controlled attenuation parameter) was detected by Fibrotouch, ultrasonic fatty liver classification was examined by color Doppler ultrasonography and also serum Acrp30, visfatin were tested by ELISA. These indexes of each group were compared and analyzed. Results The levels of CAP, TG, TC and Acrp30 in NAFL group were lower than those in NASH group, and the level of Visfatin was higher than that of group NASH, the difference was statistically significant (P〈0.05). The results of Pearson correlation analysis showed that the liver CAP value of NAFL group was moderately correlated with TG and TC (P〈0.05), it was strongly related to Acrp30 and Visfatin (P〈0.05); The liver CAP value of NASH group was moderately correlated with TG and TC (P〈0.05), it was strongly related to Acrp30 and Visfatin (P〈0.05). The liver CAP of NAFL and NASH groups was strongly correlated with the ultrasonic classification of fatty liver (P〈0.05). Conclusion CAP could be helpful for evaluation of nonalcoholic fatty liver disease, which deserve further.
