巨噬细胞炎症蛋白-1α和高敏C-反应蛋白在高血压合并阻塞性睡眠呼吸暂停低通气综合征患者中的表达及意义 被引量:9

摘要 目的探讨高血压(hypertension,HT)合并阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)患者血清中巨噬细胞炎症蛋白-1α(MIP-1α)和高敏C-反应蛋白(hs-CRP)含量变化对疾病发展和疗效预测的意义。方法选择162例受试者,其中健康受试者40例,单纯HT患者42例,OSAHS患者37例,HI合并OSAHS(HT+OSAHS)患者43例,比较各组患者血清中MIP-1α和hs-CRP含量。根据睡眠呼吸暂停低通气指数(AHI)将高HT+OSAHS组患者分为轻度、中度和重度三个亚组,比较各亚组患者血清中MIP-1α和hs-CRP含量。将HT+OSAHS患者进行氨氯地平联合替米沙坦治疗,比较治疗前后患者血清中MIP-1α和hs-CRP含量,并且观察患者临床疗效。结果 HT+OSAHS组患者血清中MIP-1α和hs-CRP含量高于其他三组,与正常健康人群受试组相比,差异具有显著性(P<0.01)。OSAHS患者病情越严重,血清中MIP-1α和hs-CRP含量越高。HT+OSAHS组患者经治疗3个月后,病情得到显著改善,血清中MIP-1α和hs-CRP含量显著降低,与治疗前相比,差异有显著性(P<0.05)。结论 OSAHS与HI的发病机制可起到协同作用,同时促使患者血清中MIP-1α和hs-CRP含量升高,因此检测血清中MIP-1α和hs-CRP含量,对于评估和预测HT+OSAHS组患者的严重程度和治疗效果有一定的临床意义。
