摘要
目的探讨双钢板三角固定治疗肱骨髁间骨折的临床疗效。方法对40例肱骨髁间骨折患者采用双钢板三角固定治疗。分别于术后6个月和12个月进行随访,观察患者的临床疗效、患侧功能恢复情况、临床疗效影响因素以及手术并发症情况。结果术后6个月,根据Mayo肘关节功能评分法评定:优14例,良19例,中5例,差2例,优良率为82.5%。术后12个月,患侧与健侧VAS评分、关节活动范围、DASH评分以及关节炎比较差异无统计学意义(P>0.05)。不同骨折类型患者的临床疗效比较差异有统计学意义(P<0.05)。术后并发症发生率25.0%,再手术率10.0%。结论双钢板三角固定能够恢复内侧柱和外侧柱的肘关节功能,是治疗肱骨髁间骨折的有效方法。
Objective To investigate the clinical curative effect of the treatment of humeral intercondylar fracture with the triangle fixation of double column plates. Methods Forty cases of humeral intercondylar fracture patients were treated with triangle fixation of double column plates therapy, the patients were followed up at 6 months and 12 months after operation, the clinical efficacy and safety of all patients, ipsilateral functional recovery, influencing fac-tors and surgery related complications were evaluated. Results After 6 months of follow-up, according to the Mayo elbow joint function score,the results showed 14 cases were the excellent, 19 cases were good, 5 cases fair, and 2 ca-ses poor, the excellent and good rate was 82. 5% . The differences of the data about 12 months postoperative side and healthy side of VAS, the scope of activities, the DASH score and arthritis had no statistical significance （P 〉0. 05）. There were significant differences in the clinical efficacy data of different fracture （ P 〈 0. 05 ） , the incidence rate of postoperative complications was 25. 0% , and the reoperation rate was 10.0%. Conclusions Triangle fixation ofdouble column plates to maintain joint congruency and medial and lateral column of the elbow joint function recovery in the treatment of humeral intercondylar fracture is a valid method.
