摘要
目的观察直肠腔内超声弹性分级法联合应变比值法诊断直肠腺瘤的临床价值。方法选取72例直肠腺瘤患者,患者均行常规及直肠腔内超声弹性成像检查,以手术病理结果为基准计算常规直肠腔内超声与直肠腔内超声弹性分级联合应变比值法诊断直肠腺瘤的准确性。结果手术病理结果:直肠腺瘤24例、直肠腺癌48例。常规直肠腔内超声诊断直肠腺瘤的敏感度75.00%、特异度72.92%、准确性73.61%;弹性分级法的敏感度95.83%、特异度83.33%、准确性87.50%;应变比值法的敏感度87.50%、特异度87.50%、准确性87.50%;弹性分级法联合应变比值法的敏感度95.83%、特异度89.58%、准确性91.67%。弹性分级法联合应变比值法的诊断准确性显著高于常规直肠腔内超声(P<0.05)。结论直肠腔内超声弹性分级联合应变比值法诊断直肠腺瘤的准确性高于常规直肠腔内超声,可为术前明确肿物性质、制定手术方案提供有力依据,具有较高的临床应用价值。
Objective To study the clinical value of elastic fractionation combined withstrain ratio method in the diagnosis of rectal adenoma. Methods 72 cases of rectal adenomawere selected. All patients underwent conventional and transrectal ultrasound elastography. Based on the Results of surgical pathology,the diagnostic accuracy of conventional transrectal ultrasound and transrectal ultrasound elastography combined with strain ratio method was analyzed. Results The surgical pathologic result showed rectal adenoma in24 cases and rectal adenocarcinoma in 48 cases. Sensitivity,specificity and accuracy of transrectal ultrasonography in the diagnosis of rectal adenomas were 75%,72.92% and 73.61%,respectively. For the elastic grading method,the corresponding figures were 95.83%,83.33% and 87.50%,respectively,for the strain ratio method,87.50%,87.50% and 87.50%,respectively,for the elastic grading combined with strain ratio method,95.83%,89.58% and 91.67%,respectively. The diagnostic accuracy of elastic grading method combined with strain ratio method was significantly higher than that of conventional transrectal ultrasound（P〈0. 05）. Conclusion The accuracy of elastic fractionation combined with strain ratio method in diagnosis of rectal adenoma is higher than conventional ultrasound. it can provide useful insight for confirming the nature of tumor mass and surgical treatment plan. It has great clinical value.
