摘要
目的观察手法治疗膝骨关节炎(knee osteoarthritis,KOA)的疗效,并探讨其作用机制。方法将60例KOA患者随机分为治疗组、对照组,每组30例,对照组在常规治疗的基础上采用随意按摩法,治疗组在常规治疗的基础上采用手法治疗,疗程结束后采用ELISA法检测患者血清胰岛素样生长因子-1(insulin-like growth factor 1,IGF-1)、白细胞介素-1(interleukin-1,IL-1)、肿瘤坏死因子-α(tumor necrosis factor alpha,TNF-α)含量,比较两组患者治疗前后视觉模拟量表(visual analogue scale,VAS)评分。结果两组患者血清IGF-1、IL-1含量与治疗前比较,差异有统计学意义(P<0.05);两组治疗前后血清IGF-1、IL-1、TNF-α含量差值比较,差异均有统计学意义(P<0.05)。两组治疗后与治疗前VAS评分比较,差异均有统计学意义(P<0.05);两组治疗后VAS评分差值比较,差异有统计学意义(P<0.05)。结论手法能改善KOA的疼痛,缓解病情,提高临床疗效,其作用机制可能与降低血清中炎性因子含量有关。
Objective To investigate the clinical effect of manipulation in the treatment of knee osteoarthritis (KOA) and the possible mechanism.Methods A total of 60 patients with KOA were randomly divided into treatment group and control group, with 30 patients in each group.The patients in the control group were given massage in addition to conventional treatment, and those in the treatment group were given manipulation in addition to conventional treatment.ELISA was used to measure the serum levels of insulin-like growth factor-1 (IGF-1), interleukin-1 (IL-1), and tumor necrosis factor-α (TNF-α), and the Visual Analogue Scale (VAS) score before and after treatment was compared between the two groups.Results Both groups had significant changes in the serum levels of IGF-1 and IL-1 after treatment (P〈0.05), and there were significant differences between the two groups in the serum levels of IGF-1, IL-1, and TNF-α before and after treatment (P〈0.05).Both groups had a significant change in VAS score after treatment (P〈0.05), and there was a significant difference in VAS score after treatment between the two groups (P〈0.05).Conclusion Manipulation can reduce the pain of KOA, alleviate patients' conditions, and improve their clinical outcome, possibly by reducing the serum levels of inflammatory factors.
