不同类型恶性淋巴瘤患者体内免疫指标水平分析 被引量:6

In vivo immunological indicators in patients with different types of malignanat lymphoma

摘要 目的:分析不同类型恶性淋巴瘤患者体内免疫指标水平。方法:选取本院收治的不同类型的恶性淋巴瘤患者69例,其中原发性结外恶性淋巴瘤患者31例(结外组),原发性结性淋巴瘤患者38例(结性组),另选取同期收治的单纯淋巴结炎患者34例作为对照组,采用流式细胞术和全自动速率散射比浊法测定各组T淋巴细胞亚群比例、免疫球蛋白(Ig)IgG、IgA、IgM及补体C3、C4含量。结果:各组间CD_3^+、CD_4^+、CD_8^+、NK细胞比例及CD_4^+/CD_8^+比较差异显著(P<0.05或P<0.01),与对照组比较,结外组和结性组CD_3^+、CD_4^+、CD_8^+、NK细胞比例及CD_4^+/CD_8^+均显著下降(P<0.05或P<0.01),且结外组CD_3^+、CD_4^+、CD_8^+及CD_4^+/CD_8^+低于结性组(P<0.05或P<0.01),结外组NK细胞比例高于结性组(P<0.01);各组间IgG、IgA、C3水平比较差异显著(P<0.05),与对照比较,结外组和结性组IgG、IgA水平显著升高,C3水平显著降低(P<0.05),且结外组与结性组之间差异显著(P<0.05),结外组和结性组IgM水平有所升高,C4有所降低,但与对照组无显著差异(P>0.05);Spearman轶相关分析显示,恶性淋巴瘤中CD_3^+、CD_4^+细胞比例与IgA、IgG、C3具有显著相关性(P<0.01)。结论:随着淋巴肿瘤严重程度的增加而机体免疫水平降低,而恶性淋巴瘤中CD_3^+、CD_4^+细胞比例与IgA、IgG、C3具有显著相关性,提示可通过检测患者体内CD_3^+、CD_4^+细胞比例或IgA、IgG、C3含量结合病理特征评价恶性淋巴瘤疾病严重程度,为临床诊断和治疗提供指导。 Objective:To analyze the levels of in vivo immunological indicators in patients with different types of malignanat lymphoma.Methods:A total of 69 patients with different types of malignanat lymphoma who were admitted in our hospital were included in the study,among which 31 had primary extranodular malignant lymphoma(extranodular group),and 38 had primary nodular malignant lymphoma(nodular group).Moreover,34 patients with pure lymphnoditis were served as the control group.FCM and full-automatic speed scattering turbidimetry were used to detect T lymphocyte subsets proportion,IgG,IgA,IgM,C3,and C4 in the three groups.Results:The change of CD3^+,CD4^+,CD8^+,NK cell proportion,and CD4^+/CD8^+among the three groups was significantly different(P〈0.05 or P〈0.01).When compared with the control group,CD3^+,CD4^+,CD8^+,NK cell proportion,and CD4^+/CD8^+in the extranodular group and nodular group were significantly reduced(P〈0.05 or P〈0.01);moreover,CD3^+,CD4^+,CD8^+,and CD4^+/CD8^+in the extranodular group were significantly lower than those in the nodular group(P〈0.05 or P〈0.01),while NK cell in the extranodular group was significantly higher than that in the nodular group(P〈0.01).The difference of IgG,IgA,and C3 levels among the three groups was statistically significant(P〈0.05).When compared with the control group,IgG and IgA levels in the extranodular group and nodular group were significantly elevated,while C3 level was significantly reduced(P〈0.05),and the difference between the extranodular group and nodular group between the two groups was statistically significant(P〈0.05).IgM level in the extranodular group and nodular group was elevated,C4 level was reduced,but the differencen with the control group was not statistically significant(P〉0.05).Spearman correlation analysis showed that CD3^+,and CD4^+were significantly correlated with IgA,IgG,and C3(P〈0.01).Conclusions:With the increasing of severity degree
