摘要
目的探讨经皮球囊扩张椎体后凸成形术(PKP)治疗老年骨质疏松陈旧性胸腰椎压缩骨折的临床效果及应用价值。方法选取我院2016年1月~2017年1月收治的27例老年骨质疏松陈旧性胸腰椎压缩骨折患者,均实施PKP治疗,对患者的临床资料进行回顾性分析,记录并比较术前术后的VAS疼痛评分(通过随访测定)、矢状面节段后凸Cobb角以及术后椎体高度(通过X线检测)等临床指标。结果 27例患者均顺利完成PKP手术。术后患者的VAS疼痛评分为(2.57±0.47)分,显著低于术前的(8.56±0.71)分,差异有统计学意义(P<0.05)。患者术前、术后的矢状面节段后凸Cobb角大小比较,差异无统计学意义(P>0.05)。患者术后的椎体前壁高度、椎体中间高度显著优于术前,差异有统计学意义(P<0.01)。结论 PKP治疗老年骨质疏松陈旧性胸腰椎压缩骨折的临床效果较好,能够显著减轻患者的疼痛,且手术创伤小,有利于提高患者生活质量,具有临床推广应用价值。
Objective To explore the clinical efficacy and application value of percutaneous balloon kyphoplasty（PKP）in the treatment of old thoracolumbar vertebrae compression fractures in elderly patients with osteoporosis.Methods 27 elderly patients with old thoracolumbar vertebrae compression fractures were enrolled in our hospital from January 2016 to January 2017.The patients were treated with PKP,the clinical data were analyzed retrospectively.The VAS pain scores（postoperative follow-up）,posterior segment kyphosis and postoperative vertebral height（by X-ray） were recorded and compared between before and after operation.Results 27 patients were successfully completed PKP operation.The VAS pain score after operation was（2.57±0.47） points,which was significantly lower than that before operation [（8.56 ±0.71） points],the difference was statistically significant（P〈0.05）.There was no significant difference between the preoperative and postoperative in the sagittal segmental kyphosis Cobb angle（P 〉0.05）.The vertebral body of anterior wall height and the middle height of the patients after operation were better than that before operation,with significant difference（P〈0.01）.Conclusion PKP has a good clinical effect in the treatment of old thoracolumbar spine compression fractures in elderly patients with osteoporosis,which significantly reduces the pain of patients and has a small surgical trauma,which is beneficial to improve the patients′ quality of life,with clinical promotion and application value.
